苓桂术甘汤联合福辛普利钠片治疗伴有肺动脉高压的慢性肺源性心脏病患者的效果分析

摘　　要：目的:研究苓桂术甘汤联合福辛普利钠片治疗伴有肺动脉高压(Pulmonary Arterial Hypertension,PAH)的慢性肺源性心脏病(Chronic Pulmonary Heart Disease,CPHD)患者的效果。方法:选择89例伴有PAH的CPHD患者,根据不同的治疗方案将其分为对照组(44例)和研究组(45例)。两组均使用低流量吸氧、抗感染等常规治疗,对照组使用福辛普利钠治疗,研究组在对照组的基础上联合苓桂术甘汤治疗。治疗3个月后,对比两组的临床疗效、超声心动图指标、血清生化指标。结果:研究组的有效率高于对照组(P <0.05),治疗后研究组的右心室射血分数(Right Ventricular Ejection Fraction,RVEF)高于对照组,右心室舒张末期内径(Right Ventricular End Diastolic Dimension,RVEDD)、肺动脉收缩压(Pulmonary Artery Systolic Pressure,PASP)低于对照组(P <0.05),治疗后研究组的N末端脑钠肽前体(N-Terminal pro-Brain Natriuretic Peptide,NT-proBNP)、内皮素-1(Endothelin-1,ET-1)低于对照组(P <0.05)。结论:苓桂术甘汤联合福辛普利钠治疗伴有PAH的CPHD患者的效果显著,相比单纯的福辛普利钠治疗能够更好的防止右心室重构及肥厚,降低肺动脉高压。Objective:To study the effect of the Linggui Zhugan decoction(苓桂术甘汤) with fosinopril sodium in the treatment of chronic pulmonary heart disease patients with pulmonary arterial hypertension.Methods:89 CPHD patients with PAH were selected and divided into the control group(44 cases) and the study group(45 cases) according to different treatment plans.Both groups were treated with low-flow oxygen inhalation,anti-infection and other conventional treatments.The control group was treated with fosinopril sodium,and the study group was treated with the Linggui Zhugan decoction more.After 3 months of treatment,the clinical efficacy,echocardiographic indexes,and serum biochemical indexes in the two groups were compared.Results:The effective rate in the study group was higher than that in the control group(P<0.05).After treatment,the right ventricular ejection fraction in the study group was higher.RVEDD and PASP were lower than those in the control group(P<0.05).After treatment,NT-proBNP and ET-1 in the study group were lower than those in the control group(P<0.05).Conclusion:The Linggui Zhugan decoction with fosinopril sodium is effective in treating CPHD patients with PAH.Compared with simple fosinopril sodium treatment,it can better prevent right ventricular remodeling and hypertrophy,and reduce PAH.
