摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者血清S100钙结合蛋白A8/A9复合物(S100A8/A9)、C-X3-C趋化因子配体1(CX3CL1)联合检测的临床价值。方法选择2019年1—12月于该院确诊的COPD患者120例,根据病程不同将其分为COPD急性加重期(AECOPD)组(53例)和COPD稳定期组(67例),另选择同期体检健康者90例作为对照组,比较3组基线资料、肺功能[第1秒用力呼气容积(FEV1)、FEV1占预计值百分比(FEV1/Pre)、FEV1/用力肺活量(FVC)]、血清S100A8/A9、CX3CL1和炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和C反应蛋白(CRP)]水平;分析COPD患者血清S100A8/A9、CX3CL1水平与肺功能、炎症因子的相关性以及对COPD的预测价值。结果与对照组比较,COPD稳定期组和AECOPD组患者的吸烟年限更长(P<0.05),血清S100A8/A9、CX3CL1、TNF-α、IL-1β和CRP水平更高(P<0.05),FEV1、FEV1/Pre和FEV1/FVC降低(P<0.05);COPD稳定期组吸烟年限短于AECOPD组,血清S100A8/A9、CX3CL1、TNF-α、IL-1β和CRP水平低于AECOPD组(P<0.05),FEV1、FEV1/Pre和FEV1/FVC高于AECOPD组(P<0.05)。COPD患者血清S100A8/A9、CX3CL1水平与血清TNF-α、IL-1β和CRP水平均呈正相关(P<0.05),与FEV1、FEV1/Pre和FEV1/FVC均呈负相关(P<0.05);血清S100A8/A9、CX3CL1单独检测及二者联合检测预测COPD的曲线下面积分别为0.743、0.751和0.854,灵敏度分别为67.37%、72.92%和82.29%,特异度分别为66.32%、67.71%、80.21%。结论COPD患者血清S100A8/A9、CX3CL1水平升高,且与肺功能、炎症因子水平相关,提示S100A8/A9和CX3CL1可能通过影响COPD患者体内炎症过程参与COPD的病情发展,血清S100A8/A9、CX3CL1联合检测对COPD的预测具有较高的价值。Objective To investigate the clinical value of combined detection of serum S100 calcium binding protein A8/A9 complex(S100A8/A9),C-X3-C chemokine ligand 1(CX3CL1)in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 120 patients with COPD who were admitted to the hospital from January 2019 to December 2019 were selected,the patients with COPD were divided into 53 patients with acute exacerbation of COPD(AECOPD group)and 67 patients with COPD stable phase(COPD stable phase group),another 90 healthy subjects were selected as the control group.The levels of baseline data,pulmonary function[forced expiratory volume in the first second(FEV1),FEV1 as a percentage of the estimated value(FEV1/Pre),FEV1/forced vital capacity(FVC)],serum S100A8/A9,CX3CL1,inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and C-reactive protein(CRP)]were compared.The correlation of serum S100A8/A9,CX3CL1 levels with lung function and inflammatory factors in COPD patients and the predictive value of serum S100A8/A9,CX3CL1 levels in COPD were analyzed.Results Compared with the control group,patients in COPD stable phase group and AECOPD group had longer smoking years(P<0.05).The serum S100A8/A9,CX3CL1,TNF-α,IL-1βand CRP levels were higher(P<0.05),and the FEV1,FEV1/Pre and FEV1/FVC were significantly reduced(P<0.05).The smoking years in COPD stable phase group was shorter than that in AECOPD group,the serum S100A8/A9,CX3CL1,TNF-α,IL-1βand CRP levels were lower than those in AECOPD group(P<0.05),and the FEV1,FEV1/Pre and FEV1/FVC were higher than those in AECOPD group(P<0.05).Serum S100A8/A9 and CX3CL1 levels were positively correlated with serum TNF-α,IL-1βand CRP(P<0.05),and negatively correlated with FEV1,FEV1/Pre and FEV1/FVC(P<0.05).The area under the curve of separate detection of serum S100A8/A9,CX3CL1 and combined detection of serum S100A8/A9 and CX3CL1 were 0.743,0.751 and 0.854 respectively,the sensitivity were 67.37%,72.92%and 82.29%respectively,and specificity were 66.32%,67.
