摘 要:目的评估N端B型利钠肽原(NT-proBNP)、S100蛋白(S100β)和5-羟色胺(5-HT)在预测脓毒症相关性脑病(SAE)中的价值。方法选取2019年9月至2020年9月该院收治的脓毒症患者64例,根据是否合并SAE分为SAE组(26例)和非SAE组(38例)。检测并比较两组患者S100β、5-HT、NT-proBNP等实验室指标,分析各指标间的相关性,运用受试者工作特征(ROC)曲线分析S100β、5-HT、NT-proBNP对SAE的预测价值。结果SAE组的PCT、5-HT、NT-proBNP和S100β水平均高于非SAE组(P<0.05)。多因素Logistic回归分析显示,S100β(OR=4.673,95%CI:2.262~7.084,P<0.001)、NT-proBNP(OR=5.254,95%CI:3.016~7.492,P<0.001)、5-HT水平升高(OR=4.375,95%CI:1.727~7.023,P<0.001)均是脓毒症患者发生SAE的危险因素。相关性分析显示,5-HT的水平与S100β、NT-proBNP呈正相关(r=0.771、0.821,P<0.001)。ROC曲线分析显示,5-HT,S100β,NT-proBNP对SAE均具有较高的预测价值,并且5-HT预测SAE的曲线下面积与S100β、NT-proBNP比较,差异无统计学意义(P>0.05)。结论5HT的水平与NT-proBNP和S100β呈正相关,3者均可以作为预测脓毒症患者发生SAE的潜在生物标志物。Objective To evaluate the value of N-terminal pro-B type natriuretic peptide(NT-proBNP),S100 proteins(S100β)and 5-hydroxytryptamine(5-HT)in predicting sepsis-associated encephalopathy(SAE).Methods A total of 64 patients with sepsis admitted to the hospital from September 2019 to September 2020 were selected.According to whether SAE was combined,they were divided into SAE group(26 cases)and non-SAE group(38 cases).The S100β,5-HT,NT-proBNP and other laboratory data were detected and compared.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of S100β,5-HT and NT-proBNP for SAE.Results The PCT,5-HT,NT-proBNP and S100βof the SAE group were significantly higher than those in the non-SAE group(P<0.05).The results of multivariate Logistic regression analysis showed that the elevated levels of S100β(OR=4.673,95%CI:2.262-7.084,P<0.001),NT-proBNP(OR=5.254,95%CI:3.016-7.492,P<0.001),5-HT(OR=4.375,95%CI:1.727-7.023,P<0.001)were risk factors for SAE in patients with sepsis.The results of correlation analysis showed that 5-HT was positively correlated with S100βand NT-proBNP(r=0.771,0.821,P<0.001).ROC curve results show that 5-HT,S100β,and NT-proBNP all had relatively high predictive value for SAE,and the area under the curve of 5-HT was not significantly different from S100βand NT-proBNP.Conclusion The level of 5HT is positively correlated with NT-proBNP and S100β,all of which can be used as potential biomarkers to predict the occurrence of SAE in patients with sepsis.
