摘 要:目的探讨中国适龄女性人群人乳头瘤病毒(HPV)疫苗覆盖率和影响因素。方法利用中国免疫规划信息管理系统收集2018-2020年全国2价、4价和9价HPV疫苗(HPV2、HPV4和HPV9)接种剂次数,估算9-45岁女性人群HPV疫苗接种率,按地区分析估算接种率与经济、卫生和教育相关指标的相关性。结果2018-2020年中国9-45岁适龄女性累计接种2159.74万剂次HPV疫苗,累计估算接种率为2.24%,以北京市、上海市和浙江省最高(8.28%、7.37%和4.68%),西藏自治区、青海省和新疆维吾尔自治区最低(0.06%、0.39%、0.46%)。北京市和上海市HPV9接种比例较高(50.98%和46.17%),西藏自治区、内蒙古自治区、青海省和新疆维吾尔自治区HPV2接种比例较高(63.25%、59.32%、50.60%和53.92%)。累计估算接种率与人均国内生产总值、人均可支配收入、人均消费支出、人均医疗保障支出、每万人高等教育在校生数和城镇人口比重呈正相关。结论中国适龄女性人群HPV疫苗接种率总体较低,并受不同地区经济、卫生和教育水平的影响。Objective To estimate coverage with human papillomavirus(HPV)vaccine among age-eligible females in China and explore factors influencing coverage.Methods We obtained vaccination data on 2-,4-,and 9-valent HPV vaccines(HPV2,HPV4,and HPV9)that were reported to the National Immunization Program Information System during 2018-2020 in China.We estimated coverage levels of HPV vaccine among 9-45-year-old females and analyzed correlation of coverage with provincial level economic,health,and educational indicators.Results A total of 21.6 million doses of HPV vaccines were administered to 9-45-year-old women during 2018-2020 in China,for an estimated full-series cumulative coverage of 2.24%.Beijing,Shanghai and Zhejiang had the highest estimated coverage levels of 8.28%,7.37%,and 4.68%,while Tibet,Qinghai,and Xinjiang had the lowest coverage levels of 0.06%,0.39%,and 0.46%.Of the three HPV vaccines,Beijing and Shanghai had the highest proportion of HPV9 vaccine use(50.98%and 46.17%),while Tibet,Inner Mongolia,Qinghai,and Xinjiang had the highest proportion of HPV2 vaccine use(63.25%,59.32%,50.60%,and 53.92%).Cumulative coverage was positively correlated with per capita gross domestic product,per capita disposable income,per capita consumption,per capita expenditure on health care,number of students in higher education per 10000 population,and proportion of urban residents.Conclusions Estimated coverage of HPV vaccine was low among age-eligible females in China and was influenced by regional economic,health,and educational levels.
