瘢痕子宫再次剖宫产子宫下段不同切口位置选择的临床效果分析

摘　　要：目的分析瘢痕子宫产妇再次剖宫产时子宫下段不同切口位置选择对临床效果的影响。方法方便选取2018年7月—2020年6月该院产科收治的瘢痕子宫再次剖宫产产妇84例,根据切口位置不同分为研究组和对照组,各42例。研究组产妇在瘢痕组织上2 cm做切口,对照组产妇在瘢痕组织下2 cm做切口,对比两组产妇的切口愈合效果、并发症发生情况、手术时间、住院时间、术中出血量。结果研究组产妇的切口愈合效果为95.24%,显著高于对照组产妇的78.57%,差异有统计学意义(χ^(2)=5.126,P<0.05);研究组产妇的并发症总发生率为2.38%,明显低于对照组产妇的21.43%,差异有统计学意义(χ^(2)=7.265,P<0.05);研究组产妇的手术时间(46.59±10.37)min、住院时间(5.34±1.26)d、术中出血量(131.06±25.12)mL等指标情况也明显优于对照组产妇相关指标,差异有统计学意义(t=5.585、8.144、12.223,P<0.05)。结论瘢痕子宫再次剖宫产子宫下段切口位置选择对治疗效果存在影响,在瘢痕组织上2 cm做切口,可以显著缩短手术、住院时间,减少术中出血量,并且降低并发症,切口愈合效果好。Objective To analyze the influence of different incision positions in the lower uterus on the clinical effect of women with scarred uterus during cesarean section.Methods Convenience selection a total of 84 women who had undergone re-cesarean section with scarred uterus from July 2018 to June 2020 in the hospital.They were divided into study group and control group according to different incision positions,with 42 cases in each group.The women in the study group made an incision 2 cm on the scar tissue,and the women in the control group made an incision 2 cm under the scar tissue.The incision healing effect,complications,operation time,hospitalization time,and intraoperative blood loss of the two groups were compared.Results The incision healing effect of the study group was 95.24%,which was significantly higher than that of the control group of 78.57%,the difference was statistically significant(χ^(2)=5.126,P<0.05);the total incidence of complications in the study group was 2.38%,which was significantly lower compared with 21.43%of the control group,the difference was significant(χ^(2)=7.265,P<0.05);the operation time of the study group was(46.59±10.37)min,hospitalization time(5.34±1.26)d,and intraoperative blood loss(131.06±25.12)mL and other indicators were also significantly better than the control group maternal related indicators,the difference was statistically significant(t=5.585,8.144,12.223,P<0.05).Conclusion The choice of the incision position of the lower segment of the uterus for re-cesarean section with scarred uterus has an impact on the treatment effect.A 2 cm incision on the scar tissue can significantly shorten the operation,hospitalization time,reduce bleeding,and reduce complications,and the incision healing effect is good.
