分析对腹股沟疝行腹腔镜经腹膜前无张力疝修补术治疗的临床效果

摘　　要：目的探究对腹股沟疝行腹腔镜经腹膜前无张力疝修补术(TAPP)治疗的临床效果。方法选取2019年1月—2020年6月在该院行腹股沟疝治疗的96例患者作为研究对象。就遵从患者自主自愿的基础上予以分组,即对照组与观察组,每组48例。对照组实施标准李金斯坦无张力疝修补术,观察组则运用腹腔镜经腹膜前无张力疝修补术。分析比对两组各项术后指标及并发症发生情况。结果观察组3项术后指标肛门排气时间(7.34±1.54)h、疼痛时间(1.24±0.41)d、住院时间(1.78±0.52)d均明显短于对照组(10.84±5.12)h、(3.01±0.56)d、(3.98±0.99)d,差异有统计学意义(t=4.535、17.669、13.630,P<0.05);观察组术后并发症发生率(4.17%)显著低于对照组(18.75%),差异有统计学意义(χ^(2)=5.031,P=0.025);两组男性患者中,对照组发生2例阴囊水肿(发生率为4.35%),观察组则未发生该并发症(发生率为0.00%)。结论采用TAPP治疗成人腹股沟疝效果显著,能有效减轻患者痛感、缩短住院时间、降低患者术后并发症发生率,值得进一步应用和推广。Objective To explore the clinical effect of laparoscopic preperitoneal tension-free hernioplasty(TAPP)for inguinal hernia.Methods A total of 96 patients who underwent inguinal hernia treatment in this hospital from January 2019 to June 2020 were selected as the research objects.They were divided into control group and observation group on the basis of obeying the patients'voluntary voluntariness,with 48 cases in each group.The control group performed standard Lichtenstein Hernia Repair,and the observation group used laparoscopic preperitoneal tension-free hernia repair.Analyzed and compared the postoperative indicators and complications of the two groups.Results The 3 postoperative indexes of and exhaust time(7.34±1.54)h,pain time(1.24±0.41)d,hospital stay(1.78±0.52)d of the observation group were significantly shorter than those of the control group(10.84±5.12)h,(3.01±0.56)d,(3.98±0.99)d,the difference were statistically significant(t=4.535,17.669,13.630,P<0.05).The incidence of postoperative complications in the observation group(4.17%)was significantly lower than that in the control group(18.75%),and the difference was statistically significant(χ^(2)=5.031,P=0.025);in the two groups of male Patients,there were 2 cases of scrotal edema in the control group(incidence rate of 4.35%),but no such complication occurred in the observation group(incidence rate of 0.00%).Conclusion TAPP has a significant effect in the treatment of adult inguinal hernia,which can effectively reduce the pain of patients,shorten the length of hospitalization,and reduce the incidence of postoperative complications.It is worthy of further application and promotion.
