围术期胰岛素泵强化血糖控制治疗对老年糖尿病患者手术切口感染预防的效果

摘　　要：目的给予老年糖尿病手术患者在围术期应用胰岛素泵强化血糖控制治疗,对其手术切口感染预防情况进行对比。方法选择该院2019年1月—2021年6月患者中160例老年糖尿病手术患者作为研究对象,随机纳入研究组80例、对照组80例,在围术期分别应用胰岛素泵强化血糖控制治疗或常规注射胰岛素治疗,分析并对比两组患者的治疗效果。结果治疗后研究组患者的空腹血糖为(7.35±2.02)mmol/L,餐后2 h血糖为(9.18±1.96)mmol/L,血糖达标时间(2.64±1.15)d,胰岛素用量(33.39±5.36)U/d,白细胞计数为(9.22±1.16)×109/L,C反应蛋白为(24.88±7.84)mg/L,感染控制时间为(5.56±1.85)d,低血糖发生率为2.50%,并发症发生率为3.75%,各项指标情况均优于对照组患者,差异有统计学意义(P<0.05)。结论给予老年糖尿病手术患者在围术期应用胰岛素泵强化血糖控制治疗,可以有效控制其血糖水平以及感染指标水平,同时感染控制时间、血糖达标时间缩短,且并发症与胰岛素用量更少,可见围术期胰岛素泵强化血糖控制治疗在老年糖尿病患者手术切口感染预防中的应用效果良好。Objective To apply insulin pump to intensive blood glucose control treatment for elderly diabetic patients during perioperative period,and compare their surgical incision infection prevention.Methods Selected 160 elderly diabetic patients from the hospital from January 2019 to June 2021 as the research objects,and 80 cases in the study group and 80 cases in the control group were randomly included.In the perioperative period,insulin pumps were used to intensive blood glucose control therapy or conventional insulin injections,and the therapeutic effects of the two groups of patients were analyzed and compared.Results After treatment,the fasting blood glucose of the study group was(7.35±2.02)mmol/L,the 2 h postprandial blood glucose was(9.18±1.96)mmol/L,the blood glucose reaching target time was(2.64±1.15)d,and the insulin dosage was(33.39±5.36)U/d,white blood cell count was(9.22±1.16)×109/L,C-reactive protein was(24.88±7.84)mg/L,infection control time was(5.56±1.85)d,hypoglycemia incidence was 2.50%,complications incidence was 3.75%,and all indicators were better than those in the control group,the difference were statistically significant(P<0.05).Conclusion The perioperative application of insulin pump to intensive blood glucose control treatment for elderly diabetic patients during perioperative period can effectively control their blood glucose levels and infection index levels.At the same time,the infection control time and the time for blood glucose to reach the standard are shortened,and the complications and insulin dosage are less.Insulin pump intensive blood glucose control therapy during operation has a good effect in the prevention of surgical incision infection in elderly diabetic patients.
