摘　　要：目的探讨静脉血涂片图文报告在基层医院血液病筛查及辅助临床分级诊疗中的应用价值。方法选择2016年1月—2020年5月于该院就诊的5000例血常规检查结果异常的患者作为研究对象,其中白细胞异常3000例、红细胞及血红蛋白异常600例,血小板异常1400例作为观察指标,结合临床治疗效果,判别静脉血涂片图文报告在基层医院血液病筛查及辅助临床分级诊疗中的应用价值。结果就诊的5000例血常规检查结果异常的患者利用静脉血涂片筛查出感染性患者1000例,贫血患者300例,血小板减少患者500例,包含慢性贫血50例,缺铁性贫血25例,溶贫20例,巨幼贫24例,骨髓增生异常综合征患者15例,多发性骨髓瘤患者15例,淋巴瘤患者30例,白血病初诊患者18例,特发性血小板减低10例。结论静脉血涂片图文报告的应用,为临床感染性疾病和血液病提供了辅助诊断指标,明显增加了临床循证医学及辩证医学的依从性,提高了临床诊断效率,在基础医院分级诊疗中临床价值显著。Objective To explore the application value of venous blood smear graphic report in basic hospital blood disease screening and auxiliary clinical graded diagnosis and treatment.Methods We selected 5000 patients with abnormal blood test results in the hospital from January 2016 to May 2020 as the research subjects,including 3000 cases of white blood cell abnormalities,600 cases of red blood cell and hemoglobin abnormalities,and 1400 cases of platelet abnormalities as observation indicators.Combined with the clinical treatment effect,the application value of graphic reports of venous blood smears in the screening of blood diseases and auxiliary clinical graded diagnosis and treatment in primary hospitals can be judged.Results 5000 patients with abnormal blood test results used venous blood smears to screen 1000 infectious cases,300 anemia cases,and 500 thrombocytopenia cases,including 50 chronic anemias,25 cases of iron deficiency anemia,20 cases of anemia,24 cases of megalophobia,15 cases of myelodysplastic syndrome,15 cases of multiple myeloma,30 cases of lymphoma,18 cases of newly diagnosed leukemia,10 cases of idiopathic thrombocytopenia.Conclusion The application of venous blood smear graphic report provides auxiliary diagnostic indicators for clinical infectious diseases and blood diseases,significantly increases the compliance of clinical evidence-based medicine and dialectical medicine,improves the efficiency of clinical diagnosis,and has significant clinical value in graded diagnosis and treatment in basic hospitals.
