下肢深静脉顺行造影在下肢慢性静脉疾病患者中的应用价值

摘　　要：目的探讨下肢深静脉顺行造影在下肢慢性静脉疾病(CVD)患者中的应用价值。方法选取2020年1月至2021年4月于福建医科大学附属第一医院就诊的850例(1300条患病下肢)以下肢静脉曲张为首发症状的就诊患者,患者入院后均常规行下肢深静脉顺行造影,根据影像学表现对静脉曲张的病因进行分析和分类,并根据不同病因制定个体化治疗方案。结果1300条患病下肢的造影成功率为100%,其中原发性下肢深静脉瓣膜功能不全870条(66.9%),单纯浅静脉曲张98条(7.5%),髂静脉压迫综合征227条(17.5%),血栓后综合征93条(7.2%),其他疾病(原发性下肢深静脉无瓣膜症、先天性静脉畸形骨肥大综合征、布加综合征)12条(0.9%)。结论下肢深静脉顺行造影具有操作简单、创伤小、准确率高的优势,有助于明确CVD的病因,对临床治疗方案的选择具有指导意义。Objective The application value of antegrade venography of lower extremity deep vein in patients with chronic venous disease(CVD)of lower extremity was investigated.Methods 850 patients(1300 sick lower extremities)with varicose veins of lower extremity as the first symptom in the First Affiliated Hospital of Fujian Medical University from January 2020 to April 2021 were selected.After admission,routine antegrade venography of lower extremity deep vein was performed and then the causes of varicose veins were analyzed and classified based on the imaging manifestations,and further individualized treatment plans were formulated according to different causes.Results The success rate of 1300 sick lower extremities venography was 100%.Among them,870 cases(66.9%)were primary lower extremity deep venous valve insufficiency,98 cases(7.5%)were simple superficial varices,227 cases(17.5%)were iliac vein compression syndrome,93 cases(7.2%)were post thrombosis syndrome,and 12 cases(0.9%)were other diseases such as primary deep venous valveless disease,Klippel-Trenaunay syndrome and Budd Chiari syndrome.Conclusion Antegrade venography of lower extremity deep vein has the advantages of simple operation,small trauma and high accuracy,which helps to clarify the etiology of CVD of lower extremity,and has guiding significance for the selection of clinical treatment scheme.
