佛山市地区妊娠早期妇女甲状腺功能参考值区间的研究

摘　　要：目的:探讨佛山市地区正常妊娠早期妇女甲状腺功能指标分布特征,建立佛山市地区妊娠早期妇女甲状腺功能参考值区间。方法:采用随机、多中心临床试验方法,选择对象为2019年1月-2021年1月期间在佛山市当地医院的产科门诊建档并分娩的健康妇女,共计2007人,采用化学发光免疫分析法测定血清促甲状腺激素(TSH)、游离甲状腺素(FT4)、甲状腺过氧化物酶抗体(TPOAB)和甲状腺球蛋白抗体(TGAB)的水平,建立早期妊娠TSH、FT4的正常参考范围。结果:佛山地区妊娠早期妇女甲状腺功能指标FT4正态性检验,参考值区间为7.46-19.82 pmol/L,TSH呈偏态分布,参考值区间为1.66-1.76 uIU/ml,与试剂盒标称参考值区间及美国甲状腺协会(ATA)指南推荐的正常参考范围之间均存在一定的差异。结论:建立佛山市地区妊娠妇女特异性的甲状腺功能参考范围,有利于减少本地区妊娠女性甲状腺功能异常的漏诊和误诊,将为及时诊断和治疗本地区妊娠甲状腺疾病提供一定的参考作用。Objective:To investigate the distribution characteristics of thyroid function indicators of early pregnancy in the Foshan region,and to establish a reference interval of thyroid function of them. Methods:A total of 2007 women of early pregnancy by multicenter,randomized clinical research,establishing the cards in the department of gynecology and obstetrics in these hospitals in Foshan region from January 2019 to July 2021 were included in the study. Serum thyrotrophin(TSH),free thyroxine(FT4),thyroid peroxidase antibody(TPOBA)and thyroglobulin antibody(TGAB)levels were measured by using the chemiluminescent immunoassay,and to establish the normal reference ranges of TSH and FT4 in early pregnancy in the Foshan region. Results:The data of FT4 with normal distribution,the distribution situation of serum FT4 was ranged from 7.46 to 19.82 pmol/L;The data of TSH showed skewed distribution,the distribution situation of serum TSH was ranged from 1.66 to 1.76 mIU/L. Which were different from the nominal reference values of the kit and the normal reference ranges recommended by the American Thyroid Association(ATA)guidelines.Conclusion:The reference range of specific thyroid function for the early pregnancy in Foshan region are established,which is conducive to reduce the missed diagnosis and misdiagnosis of thyroid function,and will play a certain role in the timely diagnosis and treatment of thyroid disease with pregnancy in local area.
