摘  要:目的:对福建省农村高血压患者免费用药政策进行经济学评价。方法:以福建省已实行高血压免费用药政策的A县为政策组,未实行政策的B市为对照组,建立Markov模型估算政策长期实施的成本-效用,利用双重差分模型实证分析政策实施对患者的门诊及住院费用等疾病负担的影响。结果:Markov模型回乘分析结果显示,政策组的经济效益(3 415.40元/QALYs)低于阈值标准(49 351元/QALYs)是优势的方案选择;双重差分模型结果显示,与对照组相比,政策地区高血压患者的门诊及住院费用均有一定程度的下降。结论:福建省高血压免费用药政策的实施具有长期的经济效益,能够降低患者的疾病经济负担,具备进一步推广的价值。Objective:To evaluate the policy of free medication for rural hypertension patients in Fujian province.Methods:Taking county A in Fujian Province where the policy of free medication for hypertension has been implemented as the policy group,and group B where the policy has not been implemented as the control area,a Markov model is established to estimate the cost-utility of the long-term implementation of the policy,and the Difference-in-Difference(DID) model is used to empirically analyze the impact of policy implementation on the burden of diseases such as outpatient and hospitalization expenses of patients.Results:The results of Markov model multiplication analysis show that the economic benefits of the policy group(3 415.40 yuan/QALYs) are lower than the threshold standard(49 351 yuan/QALYs),which is an advantageous option;the results of the DID model show that compared with the control areas,the outpatient and hospitalization costs of hypertension patients in the policy group have been decreased to a certain extent.Conclusion:The implementation of the policy of free medication for hypertension in Fujian Province has long-term economic benefits,which can reduce the economic burden of patients’ disease,and has the value of further promotion.
