白花香莲解毒颗粒促进慢性乙型肝炎HBeAg血清学转换及对血清Furin表达的临床研究

摘　　要：目的:探讨白花香莲解毒颗粒促进慢性乙型肝炎乙肝病毒e抗原(HBeAg)血清学转换及对血清弗林蛋白酶(Furin)表达的影响。方法:收集2018年7月—2019年6月在广西中医药大学第一附属医院就诊的120例HBeAg阳性的慢性乙型肝炎(CHB)患者和20例健康人的临床资料,将120例HBeAg阳性CHB患者随机分为对照组和治疗组,对照组给予恩替卡韦分散片,0.5 mg/次,1次/d,空腹口服,治疗组在对照组的基础上加服白花香莲解毒颗粒,1袋/次,2次/d,冲服。两组的观察时间均为48周,分别记录治疗组和对照组治疗前、后乙肝病毒脱氧核糖核酸(HBV DNA)载量、血清中HBeAg和Furin的表达水平及不良反应事件。结果:治疗12、24、48周时,两组患者血清HBV DNA载量均较治疗前显著下降,差异有统计学意义(P<0.05)。其中治疗12、24周,对照组血清中HBV DNA载量低于对照组,差异有统计学意义(P<0.05)。治疗48周,对照组HBeAg血清学转换和HBeAg转阴率均高于对照组,差异有统计学意义(P<0.05)。治疗24、48周,治疗组血清Furin表达量均低于对照组,差异有统计学意义(P<0.05)。结论:白花香莲解毒颗粒能够有效地抑制HBV DNA复制,促进HBeAg血清学转换,其作用机制可能与下调Furin表达有关。Objective:To explore the effect of Baihua Xianglian Jiedu Granule on promoting hepatitis B virus e antigen(HBeAg)seroconversion in chronic hepatitis B(CHB)and its influcene to the serum expression of Furin.Methods:120 cases of HBeAg-positive CHB patients were randomly divided into the control group and the treatment group,another 20 healthy people were taken as the blank control.The control group was treated with Entecavir dispersible tablets(0.5 mg/time,once a day);on which basis,the treatment group was also treated with Baihua Xianglian Jiedu Granule(1 bag/time,2 times/day).After 48 weeks of the treatment,the hepatitis B virus deoxyribonucleic acid(HBVDNA)load,serum expressions of HBeAg and Furin,as well as adverse events were observed before and after the treatment in the treatment group and the control group.Results:After 12,24,and 48 weeks of the treatment,the serum levels of HBVDNA were significantly lower than those before the treatment in the two groups(P<0.05);among which,the serum levels of HBVDNA were lower in the treatment group than those in the control group after 12 and 24 weeks of the treatment(P<0.05).After 48 weeks of the treatment,the rate of HBeAg seroconversion and HBeAg negative conversion were significantly higher in the treatment group than those in the control group(P<0.05).After 24 and 48 weeks of the treatment,the serum expressions of Furin were lower in the treatment group than those in the control group(P<0.05).Conclusion:Baihua Xianglian Jiedu Granule can effectively inhibit HBVDNA replication and promote HBeAg seroconversion,which mechanism may be related to the down-regulation of Furin expression.
