补气活血方联合静脉补液对妊娠晚期羊水偏少的疗效观察和胎膜水通道蛋白表达的影响  

Effect of Buqi Huoxue Formula Combined with Intravenous Rehydration Therapy on Hypamnion in Late Pregnancy and Its Influence to Aquaporin Expression

摘　　要：目的:分析补气活血方联合静脉补液对治疗妊娠晚期羊水偏少的临床疗效和胎膜水通道蛋白(AQP)表达的影响。方法:选择2017年1月—2019年12月年上海中医药大学附属曙光医院住院妊娠晚期羊水偏少的孕妇70例,根据随机数表法,分为治疗组35例和对照组35例。对照组给予基础补液支持治疗,治疗组在对照组的基础上口服中药补气活血方,均连续治疗14 d。比较治疗前后临床疗效、中医证候积分、羊水指数(AFI)、脐动脉收缩期峰值流速/舒张期流速(S/D)、妊娠结局。对妊娠胎膜组织中AQP3与AQP8蛋白进行蛋白印迹测定,比较治疗组与对照组蛋白表达差异。结果:治疗组的总有效率为82.86%(29/35),对照组的总有效率54.29%(19/35),两组比较,差异有统计学意义(P<0.05);治疗组的中医证候积分明显低于对照组(P<0.05);与对照组相比,治疗组的AFI更高(P<0.01),S/D值明显下降(P<0.01);患者妊娠时,治疗组不良妊娠结局率为8.57%(3/35),明显低于对照组25.71%(9/35)的不良妊娠结局率为(P<0.05)。相较对照组,治疗组的AQP8蛋白表达更高(P<0.05),而AQP3蛋白表达无明显差异。结论:补气活血方联合静脉补液疗法能够有效治疗妊娠晚期羊水偏少,降低妊娠不良结局发生率,提高临床疗效,这可能与其能够促进AQP8蛋白表达机制密切相关。Objective:To analyze the clinical effect of Buqi Huoxue Formula combined with intravenous rehydration therapy on hypamnion in late pregnancy and its influence to aquaporin(AQP)expression in fetal membranes.Methods:70 pregnant women with hypamnion in late pregnancy were randomly divided into the treatment group(n=35)and the control group(n=35).The control group was given basic support treatment;on which basis,the treatment group was also treated with Buqi Huoxue Formula.After 14 days of continuous treatment,clinical efficacy,TCM syndrome scores,amniotic fluid index(AFI),blood flow parameters of systolic and diastolic velocity ratio(S/D)and pregnancy outcome were observed and compared in the two groups.The proteins of AQP3 and AQP8 in fetal membranes were determined by Western blot method,and the differences in the proteins expressions were compared between the treatment group and the control group.Results:The total effective rate was 82.86%(29/35)in the treatment group,which was significantly higher than 54.29%(19/35)in the control group(P<0.05).The TCM syndrome scores were significantly lower in the treatment group than those in the control group(P<0.05).Compared to that in the control group,the AFI was higher(P<0.01),and the S/D value decreased significantly in the treatment group(P<0.01).The abnormal pregnancy outcome was 8.57%(3/35)in the treatment group,which was significantly lower than 25.71%(9/35)in the control group(P<0.05).The protein expression of AQP8 was higher in the treatment group than that in the control group(P<0.05);whereas there was no statistical difference in the protein expression of AQP3 between the two groups.Conclusion:Buqi Huoxue Formula combined with intravenous rehydration therapy can effectively treat hypamnion in late pregnancy,it can reduce abnormal pregnancy and improve clinical efficacy,which may be closely related to promoting the protein expression of AQP8.
