导丝扩张钳技术行经皮微创气管切开术在神经重症患者中的临床应用研究

摘　　要：目的分析导丝扩张钳技术行经皮微创气管切开术在神经重症患者中的临床应用效果。方法选取2017年8月至2020年8月于本院收治的神经重症患者70例作为研究对象,按照治疗方案的不同分组,其中行传统经皮气管切开术的患者纳入对照组,行导丝扩张钳技术行经皮微创气管切开术患者纳入观察组,比较两组治疗效果。结果观察组平均术中出血量为(7.42±2.45)mL,切口长度为(2.16±0.58)cm,手术时间为(10.24±3.68)min;对照组平均术中出血量为(15.24±2.68)mL,切口长度为(4.97±0.47)cm,手术时间为(23.35±4.58)min,两组各指标比较差异均具有统计学意义(P<0.05)。观察组术后出现皮下气肿1例,对照组术后切口出血3例,术后皮下气肿2例,切口感染1例,两组并发症发生率比较差异具有统计学意义(P<0.05)。结论神经重症患者急救期间,应用导丝扩张钳技术行经皮微创气管切开术创伤性较小、时间短、安全性高,值得临床推广应用。Objective To analyze the clinical effect of percutaneous minimally invasive tracheotomy with wire guide expansion forceps in patients with severe neurosis.Methods From August 2017 to August 2020,70 patients with severe neurosis were selected,depending on the treatment protocol,patients with traditional percutaneous tracheotomy were included in the control group,transdermal minimally invasive tracheotomy was performed by wire-tapping forceps in the observation group,the therapeutic effect of the two groups was compared.Results The average intraoperative blood loss in the observation group was(7.42±2.45)mL,the incision length was(2.16±0.58)cm,and the operation time was(10.24±3.68)min;the average intraoperative blood loss in the control group was(15.24±2.68)mL,the incision length was(4.97±0.47)cm,and the operation time was(23.35±4.58)min,the difference between the two groups was statistically significant(P<0.05).There was 1 case of subcutaneous emphysema in the observation group,there was 3 cases of postoperative incision bleeding in the control group,2 cases of postoperative subcutaneous emphysema,and 1 case of incision infection,the difference in the incidence of complications between the two groups was statistically significant(P<0.05).Conclusion During the first aid,small trauma,short time and high safety of percutaneous minimally invasive tracheotomy using wire guide expansion forceps,it has clinical application value.
