分析腹腔镜穿孔修补术对老年胃溃疡合并胃穿孔的疗效

摘　　要：目的探究腹腔镜穿孔修补术治疗老年胃溃疡合并胃穿孔的临床疗效。方法回顾性分析2016年5月至2018年6月本院收治的134例老年胃溃疡合并胃穿孔患者,将运用开腹修补术治疗的67例患者归为对照组,将运用腹腔镜穿孔修补术治疗的67例患者归为研究组,比较两组治疗效果、手术相关指标、并发症发生率。结果术后,研究组治疗总有效率为97.01%,明显高于对照组的86.57%(P<0.05);术中,研究组手术时间短于对照组(P<0.05),出血量少于对照组(P<0.05);术后,研究组胃肠道功能恢复时间、首次排气时间均短于对照组(P<0.05);研究组并发症发生率为2.99%,低于对照组的13.43%(P<0.05)。结论腹腔镜下穿孔修补术治疗老年胃溃疡合并胃穿孔患者的疗效显著,手术时间短、恢复快,且安全性高。Objective To explore the clinical effect of laparoscopic perforation repair in the treatment of elderly gastric ulcer combined with gastric perforation.Methods A retrospective analysis of 134 elderly patients with gastric ulcer and gastric perforation admitted to our hospital from May 2016 to June 2018.67 patients treated with laparotomy were classified as the control group,and laparoscopic perforation repair was used.The 67 patients treated were classified into the study group,and the treatment effects,surgery-related indicators,and complication rates were compared between the two groups.Results After operation,the total effective rate of treatment in the study group was 97.01%,which was significantly higher than the control group's 86.57%(P<0.05);during the operation,the operation time of the study group was shorter than that in the control group(P<0.05),and the blood loss was less than control group(P<0.05);postoperatively,the recovery time of gastrointestinal function and the first exhaust time in the study group were shorter than those in the control group(P<0.05);the complication rate in the study group was 2.99%,which was lower than 13.43%in the control group(P<0.05).Conclusion Laparoscopic repair of perforation is effective in treating elderly patients with gastric ulcer combined with gastric perforation.The operation time is short,the recovery is quick,and the safety is high.
