脂肪肝对慢性乙型肝炎患者应用阿德福韦酯抗病毒治疗效果的影响分析

摘　　要：目的研究脂肪肝对慢性乙型肝炎患者应用阿德福韦酯抗病毒治疗效果的影响。方法选取2018年2月至2019年10月于本院诊治的慢性乙型肝炎患者180例作为研究对象,根据患者是否合并脂肪肝分为两组,每组90例。对照组为单纯慢性乙型肝炎患者,观察组为慢性乙型肝炎合并脂肪肝患者,根据观察组患者的肝脂肪病变程度评估分为轻度组(n=56)和中重度组(n=34),所有患者均给予阿德福韦酯抗病毒治疗,比较各组患者HBeAg转阴率、HBV DNA转阴率、ALT/AST复常率及血清学与生化学指标等。结果观察组HBeAg转阴率、HBsAg转阴率、HBV DNA转阴率和ALT/AST复常率均低于对照组(P<0.05);轻度组患者HBeAg转阴率、HBsAg转阴率、HBV DNA转阴率和ALT/AST复常率均高于中重度组(P<0.05);治疗前,轻度组患者ALT、AST、GGT、ALP水平均低于中重度组(P<0.001)。结论脂肪肝对慢性乙型肝炎患者应用阿德福韦酯抗病毒治疗效果会产生一定影响,随着肝脂肪病变程度加重,阿德福韦酯的抗病毒治疗效果越差。Objective To study the effect of fatty liver on the antiviral treatment of adefovir ester in patients with chronic hepatitis B.Methods 180 patients with chronic hepatitis B who were diagnosed and treated in our hospital from February 2018 to October 2019 were selected as the research subjects.They were divided into two groups according to whether the patients had fatty liver or not,with 90 cases in each group.The control group was simple chronic hepatitis B patients,and the observation group was chronic hepatitis B patients with fatty liver.At the same time,the observation group was divided into mild group(n=56)and moderate to severe group(n=34),all patients were given adefovir ester antiviral treatment,and the HBeAg conversion rate,HBV DNA conversion rate,ALT/AST normalization rate,and serological and biochemical indexes of each group were compared.Results The rate of HBeAg turns shade,HBsAg turn overcast rate,HBV DNA and ALT/AST after often rate comparison in the observation group were lower than the control group(P<0.05);mild group of patients with HBeAg overcast rate,HBsAg turn overcast rate,HBV DNA and ALT/AST after often rate were higher than in moderate to severe group(P<0.05);before treatment,the ALT,AST,GGT and ALP levels of mild group were less than moderate to severe group(P<0.001).Conclusion Fatty liver in patients with chronic hepatitis B application adefovir ester antiviral treatment effect have an impact,as the fat liver lesion degree aggravating,adefovir ester of antiviral treatment effect is poorer.
