摘　　要：目的研究Trophinin在辅助生殖技术(ART)后输卵管异位妊娠绒毛组织中的表达与意义。方法回顾性选取2016年1月至2018年12月因输卵管异位妊娠(tubal pregnancy,TP)于广西医科大学第一附属医院住院行患侧输卵管部分切除术的患者52例,其中自然受孕所致TP 27例,体外受精-胚胎移植(IVF-ET)术后所致TP 25例。与此同时,笔者还收集患者的年龄、孕次、产次、孕周、异位妊娠史、血清β亚基人绒毛膜促性腺激素(β-HCG)水平等相关临床资料进行分析和比较。用免疫组化技术,分别检测自然受孕和IVF-ET术后输卵管异位妊娠绒毛组织中Trophinin表达情况;应用Image pro-plus 6.0软件对Trophinin蛋白表达结果进行半定量分析。结果Trophinin蛋白分子在两组胚胎绒毛组织中均有表达,且主要表达于合体滋养层细胞的细胞浆中。Trophinin在IVF-TP组的表达明显高于自然受孕所致TP组,平均光密度值分别为(0.074±0.464)和(0.027±0.030),差异有统计学意义。同时笔者还注意到,与自然受孕组相比,血清β-HCG在IVF-ET组水平更高,分别为(4062.29±2312.70)mIU/mL和(7346.24±2543.14)mIU/mL,差异有统计学意义。结论Trophinin在IVF-ET术后输卵管异位妊娠中表达升高,表明Trophinin可能参与了ART后胚胎异常着床的分子调控。Objective:To study the expression and significance of Trophinin in villi of tubal ectopic pregnancy after assisted reproductive technology(ART).Methods:52 patients who were hospitalized in the First Affiliated Hospital of Guangxi Medical University with partial salpingectomy for tubal ectopic pregnancy(TP)from January 2016 to December 2018 were retrospectively selected.Among which,27 cases of TP were caused by natural conception and 25 cases of TP were caused by IVF-ET.At the same time,we also collected relevant clinical data such as the patients'age,pregnancy,parity,gestational week,history of ectopic pregnancy,serumβhuman chorionic gonadotropin(β-HCG)for analysis and comparison.Immunohistochemical technique was used to detect Trophinin expression in the villus tissue of tubal ectopic pregnancy after natural conception and IVF-ET;Image pro-plus 6.0 software was used to semi-quantitatively analyze Trophinin protein expression results.Results:Trophinin protein molecules were expressed in two groups of embryonic villi tissues,and mainly expressed in the cytoplasm of syncytiotrophoblast cells.The expression of Trophinin in the IVF-TP group was significantly higher than that in the TP group caused by natural conception.The average optical density values were(0.074±0.464)and(0.027±0.030),respectively.The difference was statistically significant.At the same time,we also noticed that the level of serumβ-HCG in the IVF-ET group was higher than that of the natural conception group,which was(4062.29±2312.70)mIU/mL and(7346.24±2543.14)mIU/mL,respectively.Conclusion:Trophinin expression is increased in ectopic pregnancy after IVF-ET,suggesting that Trophinin may be involved in the molecular regulation of embryo implantation after ART.
