术中使用骨科牵引床患者会阴压力性损伤的保护性应用实践评价

摘　　要：目的对比和评价骨科牵引手术患者采用两种会阴保护方法对会阴组织压力性损伤的保护性应用实践成效。方法选取本院2018年1月至2020年12月168例术中使用牵引床的骨科手术患者,依据术中针对会阴压疮的保护措施不同分为两组,其中84例接受一次性纱布垫保护会阴组织的患者为对照组,84例使用专利产品改良式成人纸尿裤保护会阴组织的患者为观察组。对比两组患者术中情绪反应(焦虑自评量表,SAS)、疼痛评分(语言描述评分法,DPIS)、生理应激指标变化(收缩压/舒张压,心率)、手术时间、会阴压力性损伤发生情况、舒适度和满意度评价。结果两组患者术中SAS及DPIS评分存在明显差异(均P<0.05);两组术中舒张压波动差异不明显(P>0.05);两组术中收缩压及心率比较差异明显(均P<0.05);观察组手术用时明显短于对照组(P<0.05);两组手术舒适率及满意率差异明显(均P<0.05)。结论使用牵引床的骨科患者术中使用改良式成人纸尿裤,可明显降低会阴部位疼痛及会阴压力性损伤发生率,患者心理可接受程度、舒适度及满意度明显增高,术中生理应激指标更为稳定,手术时间明显缩短。Objective:To compare and evaluate the effect of two kinds of perineal protection methods on pressure injury of perineal tissue in patients undergoing orthopedic traction surgery.Methods:Selection in our hospital between January 2018 and December 2020,168 cases of intraoperative use of traction bed orthopedic surgery patients,according to the operation for perineal protection measures of pressure ulcers in divided into two groups,84 cases in patients undergoing one-time gauze pad to protect the perineum organization for the control group,84 cases of using patent product modified adult diapers when protecting perineum organization for the observation group of patients.Intraoperative emotional response(SAS),pain score(DPIS),changes in physiological stress indicators(SBP/DBP,HR),operation time,occurrence of perineal stress injury,comfort level and satisfaction evaluation of 2 groups were compared.Results:There were significant differences in intraoperative SAS and DPIS scores between 2 groups(P<0.05).There was no significant difference in intraoperative DBP fluctuation between the two groups(P>0.05).There were significant differences in intraoperative SBP blood pressure and HR between the two groups(all P<0.05).The operation time of observation group was significantly shorter than that of control group(P<0.05).There were significant differences in surgical comfort rate and satisfaction rate between the two groups(P<0.05).Conclusion:The use of modified adult diapers in orthopedic patients using traction bed can significantly reduce the incidence of perineal pain and perineal pressure injury,significantly increase the psychological acceptance,comfort and satisfaction of patients,more stable intraoperative physiological stress index,significantly shorten the operation time.
