活血通窍汤对血瘀型慢性鼻窦炎患者鼻内镜术后鼻黏膜形态及生理功能的影响

摘　　要：目的探讨活血通窍汤对血瘀型慢性鼻窦炎患者鼻内镜术后鼻黏膜形态及生理功能的影响。方法根据治疗方法的不同将医院收治的92例血瘀型慢性鼻窦炎患者均分观察组和对照组,均择期行鼻内镜下功能鼻窦手术治疗,对照组术后予庆大霉素+生理盐水冲洗鼻腔,观察组在对照组基础上加用活血通窍汤治疗。比较2组症状积分改善情况,临床疗效,治疗前后鼻内镜下量化评分变化,鼻黏膜纤毛传输速率(MTR)及鼻腔分泌物分泌型免疫球蛋白A(sIgA)水平,术腔黏膜清洁时间、上皮化时间,以及不良反应发生情况。结果治疗12周后,2组各项症状积分、各项鼻内镜量化评分降低,MTR、sIgA均升高,且观察组各项症状积分、各项鼻内镜量化评分降低程度及MTR、sIgA升高程度均大于对照组(P<0.05,P<0.01)。观察组总有效率高于对照组,术腔黏膜清洁时间及上皮化时间均短于对照组(P<0.05,P<0.01)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论血瘀型慢性鼻窦炎患者鼻内镜术后加用活血通窍汤可改善患者鼻黏膜形态及生理功能,促进鼻黏膜功能恢复,且安全性良好。Objective To investigate effects of Huoxue Tongqiao Decoction on morphology and physiological function of nasal mucosa in patients with blood-stasis type of chronic sinusitis after endoscopic sinus surgery.Methods According to different therapeutic methods,92 patients with blood-stasis type of chronic sinusitis were divided into observation group and control group.All patients were treated with elective functional sinus surgery under nasal endoscope.Control group was rinsed with Gentamicin+physiological saline after operation,while observation group was treated with Huoxue Tongqiao Decoction on the basis of treatment for control group.Improved condition of symptom score,clinical efficacy,changes of quantitative score under nasal endoscopy,levels of nasal mucosal ciliary transferring rate(MTR)and nasal secretion secretory immunoglobulin A(sIgA)before and after treatment,cleaning time and epithelization time of surgical cavity mucosa,and incidence rate of adverse reactions were compared between two groups.Results After treatment for 12 weeks,symptom scores and quantitative scores under nasal endoscopy were significantly decreased,while MTR and SIgA levels were significantly increased in two groups;degrees of decreased symptom scores and quantitative scores under nasal endoscopy and increased levels of MTR and sIgA in observation group were more obvious than those in control group(P<0.05,P<0.01).Compared with those in control group,the total effective rate was significantly higher,while the cleaning time and epithelization time of surgical cavity mucosa were significantly shorter in observation group(P<0.05,P<0.01).There was no significant difference in incidence rate of adverse reactions between two groups(P>0.05).Conclusion The supplementation of Huoxue Tongqiao Decoction after endoscopic sinus surgery in patients with blood-stasis type of chronic sinusitis may improve the morphology and physiological function of nasal mucosa and promote the recovery of nasal mucosal function with good safety.
