超声引导喉上神经阻滞对声带肿物手术后镇痛的效果评价

摘　　要：目的:探讨超声引导喉上神经阻滞对声带肿物手术后镇痛的效果。方法:选择2020年6-12月于本院行声带肿物手术的50例患者,按照随机数字表法将其分为两组,各25例。观察组于超声引导下行喉上神经阻滞,对照组不进行喉上神经阻滞,气管插管后直接进行手术。比较两组患者视觉模拟评分法(VAS)评分、住院时间、住院费用及并发症。结果:观察组术后4、6、12、24、48 h的VAS评分分别为(2.24±1.12)、(2.13±1.05)、(2.03±0.79)、(1.73±0.54)、(1.43±0.65)分,均低于对照组的(3.68±1.05)、(3.42±1.12)、(3.06±0.85)、(2.45±0.79)、(1.98±0.83)分,观察组的住院时间(3.73±1.05)d,短于对照组的(5.14±1.28)d,观察组的住院费用(2882.07±218.56)元,低于对照组的(3994.75±436.12)元,观察组的并发症发生率为4.00%,低于对照组的32.00%,差异均有统计学意义(P<0.05)。结论:超声引导喉上神经阻滞用于声带肿物手术后镇痛效果确切,能够减少相关并发症,加快患者恢复,减轻患者经济负担,临床应用安全可靠。Objective:To investigate the effect of ultrasound-guided superior laryngeal nerve block on postoperative analgesia of vocal cord masses.Method:A total of 50 patients who underwent vocal cord mass surgery in our hospital from June to December 2020 were selected,they were divided into two groups according to random number table method,with 25 cases in each group.The observation group received superior laryngeal nerve block under ultrasound guidance,while the control group did not receive superior laryngeal nerve block,and the operation was performed directly after endotracheal intubation.Visual analogue scale(VAS)score,length of hospital stay,hospitalization cost and complications were compared between the two groups.Result:VAS scores of the observation group were(2.24±1.12),(2.13±1.05),(2.03±0.79),(1.73±0.54),(1.43±0.65)points at 4,6,12,24 and 48 h postoperatively,respectively,lower than(3.68±1.05),(3.42±1.12),(3.06±0.85),(2.45±0.79),(1.98±0.83)points of the control group,the hospitalization time of the observation group was(3.73±1.05)d,shorter than(5.14±1.28)d of the control group,the hospitalization cost of the observation group was(2882.07±218.56)yuan,lower than(3994.75±436.12)yuan of the control group,and the complication rate of the observation group was 4.00%,lower than 32.00%of the control group,the differences were statistically significant(P<0.05).Conclusion:Ultrasonicguided superior laryngeal nerve block has a definite analgesic effect after surgery for vocal cord masses,which can reduce related complications,speed up recovery of patients,reduce the economic burden of patients,and is safe and reliable in clinical application.
