不同灸量长蛇灸治疗气滞血瘀型腰椎间盘突出症的临床效果观察

摘　　要：目的:研究不同灸量长蛇灸治疗气滞血瘀型腰椎间盘突出症的临床效果。方法:选取2018年7月-2020年12月在本科就诊的腰椎间盘突出症患者90例为研究对象,按随机数字表法将其分为三组,分别为长蛇灸30、45、60 min组,简称A、B、C组,每组30例。三组患者均先给予相同的针刺治疗,然后给予不同时长的长蛇灸治疗,1次/周,连续4次为1个疗程,共2个疗程。比较三组治疗前、疗程结束、治疗后3个月的VAS疼痛评分、现有疼痛强度(PPI)、Oswestry功能障碍指数(ODI),比较三组临床疗效。结果:治疗前,三组的VAS、PPI评分以及ODI比较,差异均无统计学意义(P>0.05)。疗程结束和治疗后3个月,B、C组的VAS、PPI评分以及ODI均显著低于A组,差异均有统计学意义(P<0.05);B、C组VAS、PPI评分以及ODI比较,差异均无统计学意义(P>0.05)。A组有效率为70.00%,B组有效率为96.67%,C组有效率为93.33%,三组临床疗效比较,差异有统计学意义(P<0.05)。B、C组的有效率远高于A组,差异均有统计学意义(P<0.05);B、C组有效率比较,差异无统计学意义(P>0.05)。结论:长蛇灸45 min和60 min治疗气滞血瘀型腰椎间盘突出症疗效显著,明显缓解患者的疼痛,改善患者的功能。考虑同等疗效的情况下,长蛇灸45 min可减少因长时间俯卧引起的不适感,根据本次研究结果,推荐长蛇灸45 min用于气滞血瘀型腰椎间盘突出症的治疗。Objective:To study the clinical efficacy of different moxibustion amounts of long snake moxibustion in treating lumbar intervertebral disc herniation of qi stagnation and blood stasis.Method:A total of 90 patients with lumbar disc herniation who received medical treatment in the undergraduate department from July 2018 to December 2020 were selected as the research subjects;according to the random number table method,they were divided into three groups,long snake moxibustion group for 30 min,45 min and 60 min,referred to as group A,B and C,30 cases in each group.All patients in the three groups were given the same acupuncture treatment first,and then long snake moxibustion treatment of different time,once a week,4 times consecutively as a course of treatment,a total of 2 courses.VAS pain scores,existing pain intensity (PPI),Oswestry dysfunction index (ODI) were compared before treatment,at the end of treatment course,and 3 months after treatment among three groups,the clinical efficacy was compared among three groups.Result:Before treatment,there were no significant differences in VAS,PPI and ODI among three groups (P>0.05).At the end of treatment course and 3 months after treatment,VAS scores,PPI scores and ODI of group B and C were significantly lower than those of group A,the differences were statistically significant (P<0.05);there were no significant differences in VAS,PPI and ODI between group B and group C (P>0.05).The effective rate was 70.00% in group A,96.67% in group B and 93.33% in group C,the clinical efficacy of the three groups was statistically significant (P<0.05).The efficiency of group B and C were much higher than that of group A,the differences were statistically significant (P<0.05);there was no significant difference in effective rate between group B and group C (P>0.05).Conclusion:Long snake moxibustion for 45 min and 60 min have a significant effect on the treatment of qi stagnation and blood stasis type lumbar disc herniation,which can obviously relieve the pain of patients and improve th
