摘 要:目的探讨静脉预注小剂量甲氧明预防老年患者全身麻醉诱导期低血压的效果。方法60例老年全身麻醉患者,以随机数字表法分为对照组和观察组,每组30例。观察组用小剂量甲氧明,对照组用同等剂量生理盐水。比较两组不同时间的血压水平及不良反应发生情况。结果观察组患者进入手术室(T1)、接受全身麻醉诱导1 min(T2)、接受全身麻醉诱导3 min(T3)、气管插管3 min(T4)、气管插管6 min(T5)时的收缩压(SBP)分别为(109.47±10.52)、(108.54±10.61)、(115.80±10.45)、(112.09±11.57)、(108.42±10.38)mm Hg(1 mm Hg=0.133 kPa),舒张压(DBP)分别为(72.91±6.93)、(73.43±7.11)、(79.10±8.52)、(73.97±8.21)、(71.69±8.03)mm Hg,平均动脉压(MAP)分别为(85.14±7.03)、(86.36±8.24)、(90.62±8.33)、(88.35±9.21)、(83.19±9.35)mm Hg。对照组患者T1、T2、T3、T4、T5时的SBP分别为(109.57±10.45)、(87.16±9.10)、(100.52±10.78)、(94.38±8.16)、(89.42±7.21)mm Hg,DBP分别为(72.58±6.53)、(56.11±4.09)、(67.49±6.28)、(59.10±5.06)、(53.65±4.17)mm Hg,MAP分别为(85.42±7.14)、(66.98±5.32)、(80.52±6.45)、(71.90±6.27)、(65.19±4.51)mm Hg。T3时,观察组SBP、DBP、MAP水平高于T1时,差异有统计学意义(P<0.05);T2、T4、T5时,观察组SBP、DBP、MAP水平与T1比较,差异无统计学意义(P>0.05)。T2、T3、T4、T5时,对照组SBP和DBP、MAP水平低于T1时,差异有统计学意义(P<0.05)。T2、T3、T4、T5时,观察组SBP和DBP、MAP水平高于对照组,差异有统计学意义(P<0.05)。观察组的低血压发生率3.3%低于对照组的20.0%,差异有统计学意义(P<0.05)。结论静脉预注小剂量甲氧明能有效预防老年患者全身麻醉诱导期低血压的发生。Objective To discuss the effect of intravenous preinfusion of low-dose methoxamine on preventing hypotension during induction of general anesthesia in elderly patients.Methods A total of 60 elderly patients with general anesthesia were divided into the control group and the observation group by random number table,with 30 cases in each group.The observation group received low-dose methoxamine,and the control group received the same dose of normal saline.The blood pressure at different time points and the occurrence of adverse reactions were compared between the two groups.Results In the observation group,the systolic blood pressure(SBP)when entering the operating room(T1),1 min after general anesthesia induction(T2),3 min after general anesthesia induction(T3),3 min after tracheal intubation(T4)and 6 min after tracheal intubation(T5)were(109.47±10.52),(108.54±10.61),(115.80±10.45),(112.09±11.57)and(108.42±10.38)mm Hg(1 mm Hg=0.133 kPa),the diastolic blood pressure(DBP)were(72.91±6.93),(73.43±7.11),(79.10±8.52),(73.97±8.21)and(71.69±8.03)mm Hg,and the mean arterial pressure(MAP)were(85.14±7.03),(86.36±8.24),(90.62±8.33),(88.35±9.21)and(83.19±9.35)mm Hg.In the control group,SBP at T1,T2,T3,T4 and T5 were(109.57±10.45),(87.16±9.10),(100.52±10.78),(94.38±8.16)and(89.42±7.21)mm Hg,DBP were(72.58±6.53),(56.11±4.09),(67.49±6.28),(59.10±5.06)and(53.65±4.17)mm Hg,and MAP were(85.42±7.14),(66.98±5.32),(80.52±6.45),(71.90±6.27)and(65.19±4.51)mm Hg.In the observation group,SBP,DBP and MAP at T3 were higher than that at T1,and the difference was statistically significant(P<0.05);SBP,DBP and MAP at T2,T4 and T5 were not statistically different from that at T1(P>0.05).In the control group,SBP,DBP and MAP at T2,T3,T4 and T5 were lower than that at T1,and the difference was statistically significant(P<0.05).At T2,T3,T4 and T5,SBP,DBP and MAP of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).The incidence of hypotension 3.3%in
