醋酸泼尼松联合环磷酰胺治疗甲亢浸润性突眼的临床效果分析

摘　　要：目的分析醋酸泼尼松联合环磷酰胺治疗甲状腺功能亢进症(甲亢)浸润性突眼的临床效果。方法70例甲亢浸润性突眼患者,根据随机数字表法分为常规用药组和联合用药组,每组35例。常规用药组的患者采取丙基硫氧嘧啶治疗,联合用药组采取醋酸泼尼松联合环磷酰胺治疗。比较两组患者眼睑水肿消失时间、结膜充血消失时间、眼球活动疼痛消失时间、自发性球后疼痛消失时间、治疗效果以及治疗前后的突眼评分、白细胞介素-6水平、超敏C反应蛋白水平。结果联合用药组眼睑水肿消失时间、结膜充血消失时间、眼球活动疼痛消失时间、自发性球后疼痛消失时间均短于常规用药组,差异均具有统计学意义(P<0.05)。治疗后,两组突眼评分以及超敏C反应蛋白、白细胞介素-6均低于治疗前,且联合用药组突眼评分(1.21±0.21)分、超敏C反应蛋白(7.45±1.01)mg/L、白细胞介素-6(61.01±2.46)pg/ml均低于常规用药组的(2.45±0.56)分、(9.21±1.56)mg/L、(121.51±2.12)pg/ml,差异均具有统计学意义(P<0.05)。联合用药组治疗总有效率94.29%高于常规用药组的74.29%,差异具有统计学意义(P<0.05)。结论醋酸泼尼松联合环磷酰胺在临床治疗甲亢浸润性突眼效果确切,可降低患者的突眼评分,加速病情的恢复,缩短治疗的时间。Objective To analyze the clinical effect of prednisone acetate combined with cyclophosphamide in the treatment of invasive exophthalmos of hyperthyroidism.Methods A total of 70 patients with invasive exophthalmos of hyperthyroidism were randomly divided into conventional medication group and combined medication group according to random numerical table,with 35 cases in each group.Patients in the conventional medication group were treated with propylthiouracil,and the combined medication group was treated with prednisone acetate and cyclophosphamide.Both groups were compared in terms of disappearance time of eyelid edema,conjunctival congestion,eyeball movement pain,spontaneous retrobulbar pain,therapeutic effect,and exophthalmos score,interleukin-6,high-sensitivity C-reactive protein before and after treatment.Results The disappearance time of eyelid edema,conjunctival congestion,eyeball movement pain,and spontaneous retrobulbar pain in the combined medication group were shorter than those in the conventional medication group,and the difference was statistically significant(P<0.05).After treatment,the exophthalmos scores,high-sensitivity C-reactive protein,and interleukin-6 in the two groups were lower than those of this group before treatment;the exophthalmos score(1.21±0.21)points,high-sensitivity C-reactive protein(7.45±1.01)mg/L,and interleukin-6(61.01±2.46)pg/ml in the combined medication group were all lower than(2.45±0.56)points,(9.21±1.56)mg/L,(121.51±2.12)pg/ml in the conventional medication group;all the differences were statistically significant(P<0.05).The total effective rate of treatment 94.29%in the combined medication group was higher than 74.29%in the conventional medication group,and the difference was statistically significant(P<0.05).Conclusion Prednisone acetate in combination with cyclophosphamide has definite effect on the treatment of invasive exophthalmos of hyperthyroidism,and it can reduce the exophthalmos score,accelerate the recovery and shorten the treatment time.
