摘　　要：目的采用间接法建立西藏堆龙德庆地区人群胆固醇与三酰甘油的参考区间,并与厂商提供的参考区间进行比较,为高原地区人群高血脂的防治提供更多依据。方法选取2018年至2019年在堆龙德庆地区人民医院进行健康体检的藏族人群检测其血清三酰甘油(triglyceride,TG)与胆固醇(total cholesterol,TC)水平,通过Kolmogorov-Smirnov(K-S)检验分析其正态性。非正态数据采用BOX-COX转换为近似正态,通过四分位间距法剔除离群值后采用Hoffmann法建立人群参考区间,与厂商提供的参考区间比较参考值的变化(reference change,RCV)。采用非参数检验比较两项指标在性别之间的差异并分析血脂与年龄的关系。结果采用Hoffmann法建立西藏堆龙德庆地区藏族人群参考区间,其中,TC:男性3.75~6.85mmol/L,女性3.60~6.50mmol/L;TG:0.65~2.21mmol/L。结果与厂商提供的参考区间存在差异(相对偏差>RCV)。TG与TC均随着年龄的增长呈递增趋势。结论实验室应根据服务人群的差异建立本实验室检测项目参考区间;采用Hoffmann法建立参考区间比较符合高原地区实际情况,结果可靠,可在临床中推广应用。Objective To establish reference intervals(RIs)of triglyceride(TG)and total cholesterol(TC)among adults of Tibetan in Duilong county by an indirect method and to compare it’s performance with published RIs.Methods ALL samples were collected from 1000 healthy subjects in Duilong county.TG and TC were measured in all subjects.K-S test was applied to detect the normality of data.If data was normally distributed,the original data was transformed through BOX-COX transformation to obtain a normal distribution.Outliers were omitted by Turkey method and RIs were taken by applying Hoffmann method.The reference change(RCV)was selected to inspect the statistical significance between the calculated and published RIs.Mann-Whitney U test were applied to detect statistical significance between genders.Results In this population,RIs for TG was 0.65-2.21 mmol/L;TC in male and female population were 3.75-6.85 mmol/L and 3.60-6.50 mmol/L,respectively.Among two indexes,the difference between the calculated and published RIs were more than RCV.TG and TC level had a change tendency with aging.Conclusion Laboratory should establish the reference intervals because of the differences of population.Hoffmann method is reliable and practical to establish reference intervals for plateau population.
