行气活血汤对腹腔镜胆囊切除术术后患者IL-6、TNF-α水平及肠胃功能的影响

摘　　要：目的探究行气活血汤对腹腔镜胆囊切除术术后患者白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平及肠胃功能的影响。方法选取本院2018年6月至2019年6月收治的75例腹腔镜胆囊切除术术后患者为研究对象,遵循数字随机法将其分为参照组(n=37)和研究组(n=38)。参照组开展常规对症治疗,研究组在参照组基础上开展行气活血汤治疗。比较两组患者的炎性因子指标水平、术后胃肠功能恢复时间、术后并发症发生情况及中医证候疗效。结果术后1周,研究组患者的IL-6、TNF-α、CRP水平均低于参照组,IL-10水平高于参照组,差异有统计学意义(P<0.05)。研究组患者的胃肠蠕动恢复时间、首次肛门排气时间、首次排便时间、术后首次进食时间均短于参照组,差异有统计学意义(P<0.05)。研究组患者的术后并发症总发生率为2.63%,低于参照组的16.22%,差异有统计学意义(P<0.05)。研究组患者的中医证候疗效总有效率高于参照组,差异有统计学意义(P<0.05)。结论行气活血汤治疗腹腔镜胆囊切除术术后患者可改善其炎性因子指标水平,缩短胃肠功能恢复时间,提升疗效,同时可降低术后并发症发生率。Objective To investigate the effects of Xingqi Huoxue decoction on IL-6,TNF-αlevels and gastrointestinal function in patients after laparoscopic cholecystectomy.Methods Seventy-five patients after laparoscopic cholecystectomy admitted in our hospital from June 2018 to June 2019 were selected as the research objects,and the patients were divided into reference group(n=37)and research group(n=38)according to digital random method.The reference group was given conventional symptomatic treatment,and the research group was given Xingqi Huoxue decoction treatment on the basis of the reference group.The levels of inflammatory factors indexes,postoperative gastrointestinal function recovery time,postoperative complications and TCM syndrome efficacy were compared between the two groups.Results One week after surgery,the levels of IL-6,TNF-αand CRP in the research group were lower than those in the reference group,the IL-10 level was higher than that of the reference group,and the differences were statistically significant(P<0.05).The recovery time of gastrointestinal peristalsis,the time of first anal exhaust,the time of first defecation and the time of first postoperative feeding in the research group were shorter than those in the reference group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the research group was 2.63%,which was lower than 16.22%in the reference group,and the difference was statistically significant(P<0.05).The total effective rate of TCM syndrome efficacy in the research group was higher than that in the reference group,and the difference was statistically significant(P<0.05).Conclusion The Xingqi Huoxue decoction in the treatment of patients after laparoscopic cholecystectomy can ameliorate the levels of inflammatory factors indexes,shorten the recovery time of gastrointestinal function,improve the curative effect and reduce the incidence of postoperative complications.
