摘　　要：目的分析小儿行为听阈与短纯音听性脑干反应(tone burst auditory brainstem response,tb-ABR)阈的相关性,评价小儿行为测听及tb-ABR在0~6岁儿童听力检查时的意义。方法选取2019年7月~2020年7月川北医学院附属医院耳鼻喉科听力检查的0~6岁儿童58例,所有儿童行行为测听与tb-ABR检查,tb-ABR在0.5、1、2、4 kHz4个频率的平均值>30 dB nHL为异常组(64耳),≤30 dB nHL为正常组(52耳);58例儿童按照年龄分为A(<3岁,30例)和B(3~6岁,28例)组,分析行为听阈与tb-ABR的相关性。结果行为听阈在异常组和正常组内4个测试频率均低于tb-ABR反应阈,差异有统计学意义(P<0.05);二者的差值在正常组中<20 dB,在异常组中>20 dB,两组在0.5 kHz处的差值较1、2、4 kHz稍大。结论0~6岁儿童主观行为听阈与客观tb-ABR反应阈相关性很好,两者相结合能更好地综合评估儿童听力水平。Objective To analyze the correlation of behavioral audiometry(BA)thresholds and tone burst auditory brainstem response(tb-ABR)thresholds in children aged 0~6 years old.To evaluate the significance of BA and tb-ABR in hearing assessment of children aged 0-6 years.Methods From July 2019 to July 2020,58 children aged 0 to 6 years who went to the Otolaryngology Department of affiliated Hospital of North Sichuan Medical College for hearing examination were selected as the research objects.All the children underwent BA and tb-ABR examination.The mean value of tb-ABR at four frequencies of 0.5,1.0,2.0 and 4.0 kHz,>30 dB nHL was the abnormal group(64 ears),≤30 dB nHL was the normal group(52 ears).The 58 children were divided into Groups A(<3 years old,30 cases)and B(3~6 years old,28 cases)according to their age,and the correlation between BA and tb-ABR was analyzed.Results The BA threshold in the abnormal group and the normal group was lower than the tb-ABR response threshold at four test frequencies of 0.5,1.0,2.0 and 4.0kHz,and the differences were statistically significant(P<0.05).Moreover,the difference between them was less than 20 dB in the normal group and more than 20 dB in the abnormal group.The difference between them at 0.5 kHz was slightly larger than 1.0,2.0 and 4.0 kHz.Conclusion There is a good correlation between subjective BA threshold and objective tb-ABR response threshold in children aged 0-6 years old,and the combination of the two can better evaluate children's hearing level comprehensively.
