摘　　要：目的评价6.0/7.5 F输尿管镜联合200μm钬激光光纤治疗输尿管中下段结石的安全性及疗效。方法入组2019年1月至2019年12月期间安徽医科大学第一附属医院泌尿外科收治的90例输尿管中下段结石患者资料,随机分为细输尿管镜(6.0/7.5 F)组和为普通输尿管镜(8.0/9.8 F)组,每组45例,其中45例接受细输尿管镜联合200μm钬激光治疗,45例接受普通输尿管镜联合500μm钬激光治疗,比较两组的临床资料。结果细镜组手术成功率100%,高于普通输尿管镜组(P<0.05),且输尿管镜进镜平均时间、手术时间、术后疼痛视觉模拟评分、术后住院天数均低于普通镜组(P<0.05),两组患者在一般情况、术中并发症、住院总费用差异无统计学意义(P>0.05)。两组术后均未出现输尿管撕脱、感染性休克等严重并发症。结论细输尿管镜联合200μm光纤治疗输尿管中下段结石,进镜时间明显缩短,碎石成功率提高,手术时间短,术后并发症低、恢复快,适合临床推广。Objective To evaluate the clinical efficacy and safety of 6.0/7.5 F ureteroscopy combined with 200μm holmium laser in the treatment of ureteral calculi.Methods 90 patients with ureteral calculi admitted to our department were enrolled in this study.They were randomly divided into 6.0/7.5 F ureteroscope(URS)group and 8.0/9.8 F ureteroscope group,45 patients underwent 6.0/7.5 F URS with 200μm holmium laser;45 patients underwent 8.0/9.8 F URS with 500μm holmium laser.The data were compared.Results The success rate of operation in 6.0/7.5 F URS group was 100%,which was higher than that of 8.0/9.8 F group(P<0.05),and the ureteroscope dwelling time,intraoperative ureteral injury,operation time,postoperative complications,postoperative pain visual simulation score,and postoperative hospital stay were than better the 8.0/9.8 F group(P<0.05),there was no significant difference in the general conditions,ureteral injury and total hospitalization cost between the two groups(P>0.05).There were no serious complications such as ureteral avulsion and septic shock after operation in both groups.Conclusions 6.0/7.5 F ureteroscope combined with 200μm holmium laser for the treatment of middle and lower ureteral calculi was safe and efficent.
