剖宫产术后并发腹膜外血肿一例临床诊疗分析

摘　　要：目的:探讨一例剖宫产术后并发腹膜外血肿的病因、诊疗经过及预防措施.方法:选取2020年10月襄阳市中西医结合医院收治的一例剖宫产术后并发腹膜外血肿患者,对其临床诊疗进行分析.结果:患者行剖宫产术后因血红蛋白进行性下降,行腹部超声及CT检查考虑腹膜外血肿,遂行剖腹探查术及血肿清除术,术后6 d痊愈出院.结论:剖宫产术后并发腹膜外血肿与患者本身疾病、术中止血、缝合不当等有关,可根据临床表现及影像学检查诊断,治疗的关键在于彻底止血、充分引流及防治感染.早期预防是关键,术前充分评估、严格把握剖宫产指征,术中选择适当的手术方式、提高缝合技术、彻底止血,术后加强护理及观察.Objective:To investigate the etiology,diagnosis and treatment,and preventive measures of a case of extraperitoneal hematoma complicated by cesarean section.Methods:A case of extraperitoneal hematoma complicated by cesarean section admitted to Xiangyang Hospital of Integrated Traditional Chinese and Western Medicine in October 2020 was selected,and its clinical diagnosis and treatment were analyzed.Results:Hemoglobin decreased progressively after cesarean section in our hospital.Abdominal ultrasound and CT examination were performed to consider extraperitoneal hematoma,and exploratory laparotomy and hematoma removal were performed.She was cured and discharged 6 days after surgery.Conclusion:The complications of extraperitoneal hematoma after cesarean section are related to the patient’s disease,intraoperative hemostasis and improper suture.It can be diagnosed according to clinical manifestations and imaging examination.The key to treatment is complete hemostasis,adequate drainage and prevention and treatment of infection.Early prevention is the key,preoperative full evaluation,strict grasp of the indications of cesarean section,intraoperative selection of appropriate surgical methods,improve suture technology,complete hemostasis,and strengthen postoperative nursing and observation.
