超声引导隐神经阻滞复合喉罩全麻在膝关节置换术快速康复中的应用研究

摘　　要：目的:探讨超声引导隐神经阻滞复合喉罩全麻在膝关节置换术快速康复中的应用效果。方法:选取2019年1-12月在本院行膝关节置换术的患者60例,按照随机数字表法分为试验组(n=30)和对照组(n=30)。对照组实施喉罩置入复合全身麻醉,试验组实施超声引导隐神经阻滞复合喉罩全麻。对比两组苏醒时间、拔管时间、功能锻炼时间、术后2、24 h疼痛评分、术后不良反应发生率及追加镇痛药物情况。结果:试验组苏醒时间、拔管时间、功能锻炼时间均早于对照组,差异均有统计学意义(P<0.05);试验组术后2、24 h疼痛评分均低于对照组,差异均有统计学意义(P<0.05);两组术后不良反应发生率比较差异无统计学意义(P>0.05);试验组术后追加镇痛药物的例数少于对照组,差异有统计学意义(P<0.05)。结论:超声引导隐神经阻滞复合喉罩全麻在膝关节置换术中应用效果显著,可有效缩短苏醒时间,降低患者术后疼痛感,利于患者术后进行早期功能锻炼,促进康复。Objective:To investigate the application effect of ultrasound-guided saphenous nerve block combined with laryngeal mask anesthesia in rapid rehabilitation of knee arthroplasty.Method:A total of 60 patients with knee arthroplasty in our hospital from January to December 2019 were selected and randomly divided into the experimental group(n=30)and the control group(n=30).The control group received laryngeal mask insertion combined with general anesthesia,and the experimental group received ultrasound-guided saphenous nerve block combined with laryngeal mask general anesthesia.The recovery time,extubation time,functional exercise time,postoperative 2 h pain score,postoperative 24 h pain score,the incidence of adverse reactions and postoperative additional analgesic drugs were compared between the two groups.Result:The recovery time,extubation time and functional exercise time of the experimental group were earlier than those of the control group,and the differences were statistically significant(P<0.05);the pain scores of 2 hours and 24 hours after operation in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05);there was no significant difference in the incidence of postoperative adverse reactions between the two groups(P>0.05);the number of additional analgesic drugs in the experimental group was less than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The application effect of ultrasoundguided saphenous nerve block combined with laryngeal mask general anesthesia in knee arthroplasty is significant,which can effectively shorten the recovery time,reduce the postoperative pain of patients,facilitate early postoperative functional exercise and promote rehabilitation.
