耳镜下鼓膜置管术联合腺样体切除术治疗儿童分泌性中耳炎的效果

摘　　要：目的:探讨耳镜下鼓膜置管术联合腺样体切除术治疗儿童分泌性中耳炎的效果。方法:将本院2020年3-8月87例(101耳)分泌性中耳炎(SOM)患儿分组,按照计算机分组法分为对照组43例(49耳),给予耳镜下鼓膜置管术治疗,观察组44例(52耳)在对照组基础上给予腺样体切除术治疗,比较两组临床指标、炎症因子水平、不同频率的听力阈值、术后并发症和复发率。结果:治疗后,观察组听力恢复正常时间、耳鸣耳闷消失时间、鼓膜愈合时间、中耳积液时间均优于对照组(P<0.05);治疗4个月后,观察组TNF-α、IL-6、IL-8水平均低于对照组(P<0.05);观察组在250、500、1000 Hz频率时听力阈值均低于对照组(P<0.05);观察组术后并发症发生率(4.54%)低于对照组(20.93%),复发率(3.85%)低于对照组(18.37%)(P<0.05)。结论:耳镜下鼓膜置管术联合腺样体切除术可显著改善SOM患儿临床指标,下调炎症因子水平,改善听力情况,降低术后并发症和复发情况发生。Objective:To investigate the effect of otoscopic tympanostomy tube placement combined with adenoidectomy on secretory otitis media in children.Method:A total of 87 children(101 ears)with secretory otitis media(SOM)in our hospital from March to August 2020 were divided into two groups according to the computerized grouping method.43 patients(49 ears)in the control group were given otoscope tympanostomy treatment,44 patients(52 ears)in the observation group were given adenoidectomy treatment on the basis of the control group.The clinical indicators,the inflammatory factors levels,and different frequencies of hearing threshold,postoperative complications and recurrence rate between the two groups were compared.Result:The normal hearing recovery time,disappearance time of tinnitus and ear tightness,tympanic membrane healing time and middle ear effusion time were earlier than those in the control group at all frequencies(P<0.05).After 4 months of treatment,the levels of TNF-α,IL-6 and IL-8 in the observation group were lower than those in the control group(P<0.05).The hearing thresholds of the observation group at 250,500 and 1000 Hz were lower than those of the control group(P<0.05).The incidence of postoperative complications in the observation group(4.54%)was lower than that in the control group(20.93%),and the recurrence rate(3.85%)was lower than that in the control group(18.37%)(P<0.05).Conclusion:Otoscopic tympanostomy tube placement combined with adenoidectomy can significantly improve the clinical indicators,down regulate the levels of inflammatory factors,improve hearing,and reduce the occurrence of postoperative complications and recurrence in children with SOM.
