不同鼻饲法对心脏术后患儿呼吸循环与胃肠的影响

摘　　要：目的:探讨持续泵入鼻饲喂养法对心脏病术后患儿呼吸循环功能及胃肠的影响。方法:选取2017年10月-2018年2月在本院心脏外科行心脏手术的90例患儿为研究对象,按照随机数字表法将其分为A组(注射器推注鼻饲法)、B组(重力喂养鼻饲法)、C组(持续泵入鼻饲法),每组30例,分别观察从鼻饲喂养开始至结束后1 min患儿的心率、呼吸频率、血氧饱和度的变化及鼻饲并发症发生情况,并比较三组患儿家属的满意度。结果:B组、C组患儿心率、呼吸频率、血氧饱和度变化幅度均低于A组,差异有统计学意义(P<0.05);且B组、C组患儿并发症发生次数低于A组;三组患儿家属的护理总满意度比较,B组、C组患儿家属总满意度高于A组,且C组患儿家属总满意度高于B组,差异有统计学意义(P<0.05)。结论:重力喂养鼻饲法和持续泵入鼻饲法均有利于患儿胃肠对营养物质的吸收,保持患儿生命体征平稳,减少鼻饲并发症的发生情况,促进术后患儿康复,且持续泵入鼻饲法患儿家属的满意度更高。Objective:To investigate the effect of continuous pumping nasal feeding on respiratory and circulatory function and gastrointestinal function in children with heart disease after operation.Method:From October 2017 to February 2018,90 children undergoing cardiac surgery in our hospital were selected as the research objects and randomly divided into group A(syringe push nasal feeding method),group B(gravity feeding nasal feeding method)and group C(continuous pumping nasal feeding method),with 30 cases in each group.The changes of heart rate,respiratory rate,blood oxygen saturation and complications of nasal feeding were observed from the beginning of nasal feeding to 1 min after the end of nasal feeding,and the satisfaction of family members of the three groups were compared.Result:The rangeability of heart rate,respiratory rate and blood oxygen saturation of group B and group C were lower than those of group A.The differences were statistically significant(P<0.05).The times of complications of group B and group C were lower than those of group A.The total family satisfaction of the group B and group C was higher than that of group A,and the total family satisfaction of group C was higher than that of group B.The differences were statistically significant(P<0.05).Conclusion:Both gravity feeding nasal feeding and continuous pumping nasal feeding are beneficial to the absorption of nutrients in children’s gastrointestinal tract,keep the vital signs stable,reduce the incidence of complications of nasal feeding,and promote the rehabilitation of children after operation.And the satisfaction of the family members of the children with continuous nasal feeding was higher.
