摘　　要：目的:探讨广东省乙型病毒性肝炎疫苗接种指引在社区婴幼儿中的实施效果。方法:选取社区2017年1月-2018年6月乙型肝炎表面抗原(HBsAg)阳性孕妇所生新生儿360例,按照乙肝疫苗(酵母)接种剂量不同将其分为5μg组(180例)和10μg组(180例)。比较两组不同接种阶段的抗-HBs阳转率、HBsAb反应情况及不良反应发生情况。结果:10μg组接种后1、3、6个月抗-HBs阳转率均明显高于5μg组,差异有统计学意义(P<0.05)。10μg组接种乙肝疫苗1、2年后HBsAb阳性率均明显高于5μg组,差异有统计学意义(P<0.05)。10μg组接种后2年HBSAb无应答率、接种后1年HBsAb低应答率均低于5μg组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:广东省乙型病毒性肝炎疫苗接种指引应用于社区有利于规范和加强社区乙肝疫苗接种工作,有效地预防和控制社区乙肝的传播,产生相当的社会效益。Objective:To explore the effect of Hepatitis B vaccination guidelines in Guangdong province on community infants.Method:A total of 360 newborns born to hepatitis B surface antigen(HBsAg)positive pregnant women in the community from January 2017 to June 2018 were selected and divided into the 5μg group(180 cases)and the 10μg group(180 cases)according to different doses of hepatitis B vaccination(yeast).The anti-HBs positive conversion rate,HBsAb reaction and the occurrence of adverse reactions were compared between the two groups at different vaccination stages.Result:The anti-HBs positive conversion rate of the 10μg group were significantly higher than that of the 5μg group at 1,3 and 6 months after vaccination,and the differences were statistically significant(P<0.05).The positive rate of HBsAb in the 10μg group were significantly higher than that in the 5μg group at 1 and 2 years after vaccination,the differences were statistically significant(P<0.05).HBsAb non-response rate at 2 years after vaccination and HBsAb low response rate at 1 year after vaccination in the 10μg group were lower than those in the 5μg group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The application of the hepatitis B vaccination guidelines in Guangdong province in the community is helpful to standardize and strengthen the work of hepatitis B vaccination in the community,and effectively prevent and control the transmission of hepatitis B in the community,and produce considerable social benefits.
