摘  要:目的:探讨DCE-MRI在鼻咽癌放疗后颅底骨质复发与颅底骨质放疗后改变鉴别中的应用价值。方法:我院2016年1月—2020年1月回顾性对比分析经临床证实的鼻咽癌放疗后颅底骨质复发(简称颅底复发组)50例和颅底骨质放疗后改变(简称放疗后改变组)250例,所有患者均行常规MRI平扫及增强扫描和DCE-MRI并获得TIC,分析DCE-MRI技术对鼻咽癌放疗后颅底骨质复发诊断的敏感度、特异度、准确度。结果:208例放疗后改变为I型曲线,45例颅底骨质复发为Ⅲ型曲线,其中42例放疗后改变和5例颅底骨质复发为Ⅱ型曲线,差异有统计学意义(P=0.000)。敏感度为90.0%、特异度为83.2%、准确度84.3%。结论:D C E-MR I技术有助于鼻咽癌放疗后颅底骨质复发与颅底骨质放疗后改变的鉴别,TIC可以从微血管血流动力学及肿瘤组织间隙结构方面进行直观分析。Objective This study was aimed at evaluating the application value of dynamic-contrast enhanced magnetic resonance imaging(DCE-MRI)in distinguishing the recurrence of skull base for nasopharyngeal carcinoma(NPC).Methods January 2016 to January 2020 A retrospective analysis was performed for 50 cases of skull base bone recurrences(the skull base recurrence group)and 250 cases of post-radiotherapy skull base bone changes(the postradiotherapy change group)for clinically proven nasopharyngeal carcinoma.MRI routine scan,enhanced scan,and DCEMRI were performed for all,specificity,accuracy of DCE-MRI technique in the diagnosis of skull base bone recurrences after radiotherapy for nasopharyngeal carcinoma.Results There were 208 cases of post-radiotherapy change for type Ⅰ curve and 45 cases of skull base recurrence for type Ⅲ curve,wherein 42 cases of post-radiotherapy change and 5 cases of skull base recurrence Ⅱ curve;the difference was statistically significant(P=0.000).The sensitivity,specificity,accuracy,were 90.0%,83.2%,84.3%,respectively.Conclusionin Distinguishing the recurrence of skull base bone from the change of skull base bone after radiotherapy for nasopharyngeal carcinoma.TIC could be used for intuitive analysis from the perspectives of hemodynamics of microvessels and the interstitial structure of tumor tissues.
