TriVex旋切联合大隐静脉激光闭合术及高位结扎加剥脱术治疗下肢静脉曲张的疗效对比

摘　　要：目的比较大隐静脉激光闭合联合TriVex旋切术与大隐静脉高位结扎加剥脱术联合TriVex旋切术治疗下肢静脉曲张的疗效。方法收集2016年6月至2019年6月山东第一医科大学第一附属医院(山东省千佛山医院)收治的330例下肢静脉曲张患者临床资料,根据治疗方式的不同将患者分为激光组(n=120)和剥脱组(n=210)。激光组患者行大隐静脉激光闭合联合TriVex旋切术,剥脱组患者行大隐静脉高位结扎加剥脱联合TriVex旋切术。比较两组患者的手术情况、术后恢复时间、术后并发症发生情况和治愈率。结果激光组患者的切口数量、术中出血量均少于剥脱组患者,手术时间、术后住院时间均短于剥脱组患者,差异均有统计学意义(P<0.05);两组患者的首次下地时间比较,差异无统计学意义(P>0.05)。激光组患者皮下血肿、术后疼痛的发生率及总发生率均低于剥脱组患者,差异均有统计学意义(P<0.05);两组患者的隐神经损伤发生率比较,差异无统计学意义(P>0.05)。330例下肢静脉曲张患者的随访率为86.7%(286/330)。286例获得随访的患者中,激光组101例,剥脱组185例。两组患者的临床治愈率比较,差异无统计学意义(P>0.05)。结论大隐静脉激光闭合联合TriVex旋切术与大隐静脉高位结扎加剥脱联合TriVex旋切术均能够达到满意的疗效且效果相当,但激光闭合联合TriVex旋切术治疗下肢静脉曲张更微创,恢复快,并发症少。Objective To compare the efficacy of laser closure of great saphenous vein combined with TriVex circumcision and high ligation and stripping of great saphenous vein combined with TriVex circumcision in the treatment of varicose veins of lower limbs.Method The clinical data of 330 patients with lower extremity varicose veins treated in the First Affiliated Hospital of Shandong First Medical University(Shandong Provincial Qianfoshan Hospital)from June 2016 to June 2019 were collected.According to different treatment methods,the patients were divided into laser group(n=120)and stripping group(n=210).Patients in the laser group underwent laser closure of great saphenous vein combined with TriVex circumcision,and patients in the stripping group underwent high ligation of great saphenous vein combined with stripping and TriVex circumcision.The operation,postoperative recovery time,postoperative complications and cure rate were compared between the two groups.Result The number of incision and intraoperative bleeding in the laser group were less than those in the stripping group,and the operation time and postoperative hospital stay were shorter than those in the stripping group(P<0.05);There was no significant difference between the two groups(P>0.05).The incidence and total incidence of subcutaneous hematoma and postoperative pain in the laser group were lower than those in the stripping group(P<0.05);there was no significant difference in the incidence of saphenous nerve injury between the two groups(P>0.05).The follow-up rate of 330 patients with lower extremity varicose veins was 86.7%(286/330).Among the 286 patients who were followed up,101 cases were in the laser group and 185 cases were in the stripping group.There was no significant difference in the clinical cure rate between the two groups(P>0.05).Conclusion Laser closure combined with TriVex circumcision of great saphenous vein and high ligation and stripping of great saphenous vein combined with TriVex circumcision can achieve satisfactory curative effect,bu
