摘　　要：目的探讨彩色多普勒超声对椎动脉疾病的诊断价值。方法将2018年5月至2020年11月中国科学技术大学附属第一医院收治的42例椎动脉疾病患者作为疾病组,将同期接受检查的48例非椎动脉疾病受检者作为对照组。比较两组受检者彩色多普勒超声颅外段椎动脉的血流动力学指标,包括血流量(Q)、收缩期峰值流速(PSV)、舒张期末流速(EDV)、内径(ID)和阻力指数(RI),采用受试者工作特征(ROC)曲线分析各项血流动力学指标对椎动脉疾病的诊断价值,计算灵敏度、特异度等诊断效能指标。结果疾病组患者的Q、PSV、EDV、ID均低于对照组,RI高于对照组,差异均有统计学意义(P<0.05)。经ROC曲线分析发现,PSV、各血流动力学指标联合检查诊断椎动脉疾病的曲线下面积(AUC)分别为0.880、0.945,具有中等诊断价值;RI、Q、EDV、ID诊断椎动脉疾病的AUC分别为0.943、0.949、0.926、0.957,具有极高诊断价值。结论椎动脉疾病在彩色多普勒超声检查下可见典型表现,彩色多普勒超声对于椎动脉疾病具有较高的诊断价值,临床应用中应结合患者临床症状全面观察各项彩色多普勒超声参数的检测结果。Objective To explore the diagnostic value of color Doppler ultrasound in vertebral artery disease.Method A total of 42 patients with vertebral artery disease admitted to the First Affiliated Hospital of USTC,Division of Life Sciences and Medicine,University of Science and Technology of China from May 2018 to November 2020 were included in the disease group,and 48 patients with non-vertebral artery disease were included in the control group at the same period of examination.The color Doppler ultrasound extracranial vertebral artery hemodynamic indicators of the two groups of subjects were compared,including quantity(Q),peak systolic velocity(PSV),end of diastolic velocity(EDV),inner diameter(ID)and resistance index(RI),the diagnostic value of the hemodynamic indexes of the two groups in the diagnosis of vertebral artery disease was analyzed by receiver operating characteristic(ROC)curve,and the diagnostic potency indexes(diagnostic sensitivity and specificity)were calculated.Result Q,PSV,EDV,ID were lower than those in control group,RI in disease group was higher than that in control group(P<0.05).ROC curve showed that the area under the curve(AUC)of PSV and combined hemodynamic indexes in diagnosing disease was 0.880,0.945,respectively,with moderate diagnostic value;the AUC for RI,Q,EDV and ID was 0.943,0.949,0.926 and 0.957,respectively,with high diagnostic value.Conclusion Typical manifestations of vertebral artery disease can be seen under color Doppler ultrasound.Color Doppler ultrasound has high diagnostic value for vertebral artery disease.In clinical application,the detection results of various color Doppler ultrasound parameters should be comprehensively observed according to the clinical symptoms of patients.
