摘　　要：目的探讨遗忘型轻度认知障碍(aMCI)患者白质纤维病变并进一步评估白质改变与认知功能变化相关性。方法对12例aMCI患者及24名与患者组年龄相匹配的健康老年人进行磁共振弥散张量成像(DTI)扫描并通过基于纤维束示踪的空间统计学分析(TBSS)方法分析aMCI患者各向异性分数(FA)图。采用简易精神状态量表(MMSE)和蒙特利尔认知评估量表(MoCA)对患者的认知功能进行评定。结果与健康老年人相比,aMCI患者仅在小范围出现了FA值的降低,包括胼胝体膝部及部分左前额叶。aMCI组白质出现差异的区域的平均FA值(0.5±0.4)与MMSE及MoCA评分存在正相关(r=0.534,P<0.01;r=0.487,P=0.01)。结论胼胝体膝部及部分左前额叶的白质在aMCI阶段即可出现损害,且FA值的下降程度与认知功能损害相关。Objective To Patients with amnestic mild cognitive impairment(aMCI)are prominently characterized by memory impairment and are at high risk of developing Alzheimer′s disease(AD).The objective of this study was to identify alterations of white matter(WM)in aMCI patients,and to further evaluate the correlation between WM alterations and cognitive function changes.Methods Diffusion tensor imaging(DTI)scan was performed on 12 aMCI patients and 24 age-matched healthy elders.The anisotropy score(FA)value was analyzed by tract-based spatial statistics(TBSS)method.The cognitive function was assessed by Mini-Mental State Examination(MMSE)Scale and Montreal Cognitive Assessment(MoCA)Scale.Results Compared with the normal controls,only a small decrease in FA values was found in aMCI patients,including the genu of corpus callosum(CC)and parts of left prefrontal cortex.In the regions with WM alterations of the aMCI group,the mean FA values(0.5±0.38)was positively correlated with the MMSE(r=0.534,P<0.01)and MoCA(r=0.487,P=0.01)scores.Conclusion The study indicates that the impaired white matter of CC genu and parts of left prefrontal cortex could be found at the aMCI stage,and the degree of FA decline may be related to cognitive impairment.
