摘  要:目的探讨败血症患儿应用万古霉素后血药谷浓度的影响因素。方法采用回顾性研究方法,收集本院2015年1月至2019年11月进行血药浓度监测的败血症患儿病历资料。结果单因素线性分析结果显示,相对给药剂量、尿素、肌酐清除率和使用呋塞米等因素与万古霉素谷浓度存在相关性。多元回归线性分析显示,使用呋塞米和肌酐清除率与万古霉素谷浓度有相关性。结论临床应根据肌酐清除率和是否使用呋塞米调整败血症患儿万古霉素的给药方案。Objective To investigate the influencing factors of plasma trough concentration after the use of Vancomycin in children with septicemia.Methods A retrospective method was used for the study,and the children with septicemia who were monitored for plasma trough concentration in our hospital between January 2015 and November 2019 were included in the study.Results The results of single-factor linear analysis showed that relative doses,urea,creatinine clearance rate and use of Furosemide were correlated with Vancomycin trough concentration.Multiple regression linear analysis showed that use of Furosemide and creatinine clearance rate were significantly correlated with Vancomycin trough concentration.Further research found that the creatinine clearance rate in trough concentration lower than 5μg/ml group with was significantly higher than that in the 5-15μg/ml and above 15μg/mL groups,while the trough concentration in the Furosemide group was significantly higher than that in the non-Furosemide group.Conclusion Creatinine clearance rate is significantly negatively correlated with Vancomycin trough concentration in children with septicemia,while the use of Furosemide is significantly positively correlated with Vancomycin trough concentration in children with septicemia.The clinical use of Vancomycin in children with septicemia should be adjusted according to the creatinine clearance rate and use Furosemide.
