不同温度膀胱冲洗液对前列腺电切术患者膀胱痉挛出血的影响

摘　　要：目的研究前列腺电切术患者术后不同温度膀胱冲洗液对膀胱痉挛、膀胱出血的影响。方法选定本院2018年11月至2020年11月住院治疗的98例前列腺电切术患者,术后均进行膀胱冲洗,根据“随机数字表法”分为参照组和研究组,每组49例。参照组患者冲洗液温度是室温,研究组患者冲洗液温度是人体腋下温度,比较2组冲洗量、冲洗时间、生命体征、膀胱痉挛率、膀胱出血率、血凝块堵管率。结果研究组冲洗量低于参照组,冲洗时间短于参照组,差异均有统计学意义(P<0.05)。研究组冲洗后呼吸、心率、收缩压、舒张压均低于参照组差异均有统计学意义(P<0.05)。研究组膀胱痉挛率、膀胱出血率、血凝块堵管率均低于参照组差异均有统计学意义(P<0.05)。结论前列腺电切术患者患者术后冲洗液温度控制在人体腋下温度,可有效缩短冲洗时间,减少冲洗量,维持生命体征稳定,降低膀胱痉挛、出血等不良事件发生率。Objective To investigate the effects of bladder irrigation fluid at different temperatures on bladder spasm and bladder hemorrhage in patients undergoing transprostatectomy.Methods A total of 98 patients who underwent electrosurgical prostatectomy in our hospital from November 2018 to November 2020 were enrolled in the study,who received bladder irrigation after surgery.According to the“random number table method”,these patients were divided into control group and research group,with 49 patients in each group.The patients in control group underwent bladder irrigation with room temperature irrigation fluid,however,the patients in research group underwent bladder irrigation with axillary temperature irrigation fluid.The flushing dose,flushing time,vital signs,bladder spasm rate,bladder bleeding rate,blood clot blocking rate were observed and compared between the two groups.Results The flushing dose in research group was significantly lower than that in control group,and the flushing time was significantly shorter than that in control group(P<0.05).Moreover respiration frequency,heart rate,systolic blood pressure,and diastolic blood pressure after irrigation in research group were significantly lower than those in control group(P<0.05),and the bladder spasm rate,bladder bleeding rate,and blood clot blocking rate in research group were significantly lower than those in control group(P<0.05).Conclusion The application of axillary temperature irrigation fluid for the patients undergoing electrosurgical resection can effectively shorten the irrigation time,reduce the amount of irrigation,maintain the stability of vital signs,and reduce the incidence of adverse events including bladder spasm and bleeding.
