摘 要:目的探究γ-干扰素rs2430561位点基因多态性与肺结核易感性之间的关系。方法选择2018年6月-2020年6月于湖州市中心医院接受治疗的70例患者设为研究组,并将同期于医院体检的70名健康人设为对照组,比较两组一般资料及γ-干扰素rs2430561位点基因多态性,Logistic逐步回归分析γ-干扰素rs2430561位点基因多态性与肺结核易感性的关系。结果研究组γ-干扰素rs2430561位点AA基因型频率高于对照组,γ-干扰素rs2430561位点AT、TT基因型频率低于对照组,研究组γ-干扰素rs2430561位点A型等位基因频率较对照组高,γ-干扰素rs2430561位点T型等位基因分布频率较对照组低(P<0.05);Logistic逐步回归分析显示吸烟史、粉尘接触、有害空气接触、卡痕、肺结核家族史、肺结核接触史及γ-干扰素rs2430561位点AA基因型是肺结核患病的影响因素(P<0.05),其中γ-干扰素rs2430561位点AA基因型OR值最高;另研究组70例患者中21例抗酸痰液涂片阳性,49例抗酸痰液涂片阴性,涂片结果阳性患者γ-干扰素rs2430561位点AA基因型、携带γ-干扰素rs2430561位点A等位基因患者比例高于阴性组(P<0.05)。结论粉尘及有毒空气接触、卡痕、肺结核家族史及接触史以及γ-干扰素基因多态性AA基因型等因素均与结核病患者的患病风险之间存在明显关系,AA基因型与患者易感性及感染性差异关系明显,γ-干扰素基因多态性及受检者流行病学资料分析对临床防治结核病工作的推进具有重要作用。OBJECTIVE To explore the relationship between the polymorphism at rs2430561locus ofγ-interferon and susceptibity to pulmonary tuberculosis.METHODS Seventy patients who received treatment in Huzhou Central Hospital from Jun.2018to Jun.2020were selected as the study group,and 70healthy people who received physical examination in the hospital during the same period were selected as the control group.The general data and gene polymorphism ofγ-interferon at rs2430561locus of the two groups were compared.Logistic stepwise regression was used to analyze the relationship between the polymorphism ofγ-interferon at rs2430561locus and pulmonary tuberculosis susceptibility.RESULTS The frequency of AA genotype at rs2430561of interferonγin the study group was significantly higher than that in the control group,the frequency of AT and TT genotype at rs2430561 of interferonγwas significantly lower than that in the control group,the frequency of A allele at rs2430561of interferonγin the study group was significantly higher than that in the control group,and the distribution frequency of T allele at rs2430561locus of interferonγwas significantly lower than that of control group(P<0.05).Logistic stepwise regression analysis showed that smoking history dust exposure,hazardous air exposure,entrapment,family history of tuberculosis,tuberculosis exposure and AA genotype at rs2430561locus of gamma-interferon were the influencing factors of pulmonary tuberculosis(P<0.05),among which and the OR value of AA genotype at rs2430561gamma-interferon was the highest.In addition,21of the 70patients in the study group were positive for acid-fast sputum smear,and 49cases were negative for acid-fast sputum smear.The proportion of patients with AA genotype at rs2430561interferonγand carrying A allele at rs2430561interferonγin patients with positive smear results was significantly higher than that of the negative group(P<0.05).CONCLUSIONDust and toxic gas exposure,stick marks,family history and exposure history of tuberculosis,as well as the
