摘 要:目的探究梅毒孕妇阻断治疗结局与免疫相关因子基因多态性的关联性。方法回顾性分析2017年1月-2020年10月武汉市第八医院收治的117例梅毒妊娠产妇的临床资料,均接受阻断治疗。采用MGB TaqMan荧光探针法检测肿瘤坏死因子-α(TNF-α)基因238位点、白细胞介素-4(IL-4)基因-590位点多态性,对各基因进行Hardy-Weinburg平衡吻合度检验;根据阻断治疗后梅毒清除情况分为治疗成功组(n=80)及治疗失败组(n=37),比较两组TNF-α基因238位点、IL-4基因-590位点基因型、基因频率及不良妊娠结局发生率;根据妊娠结局将其分为正常妊娠(n=103)及不良妊娠(n=14),比较两亚组TNF-α基因238位点、IL-4基因-590位点基因型及基因频率;分析TNF-α基因238位点、IL-4基因-590位点基因多态性与梅毒妊娠孕妇阻断治疗结局及妊娠结局的关联。结果两组TNF-α基因238位点G/A、IL-4基因-590位点T/C基因型分布符合Hardy-Weinberg平衡;治疗成功组TNF-α基因238位点A基因频率、IL-4基因-590位点C基因频率高于治疗失败组(P<0.05);TNF-α基因238位点、IL-4基因-590位点基因多态性与梅毒孕妇阻断治疗结局相关(P<0.05);治疗成功组不良妊娠结局发生率低于治疗失败组(P<0.05);正常妊娠组及不良妊娠组TNF-α基因238位点、IL-4基因-590位点基因型及基因频率对比无明显差异,TNF-α基因238位点、IL-4基因-590位点基因多态性与梅毒孕妇妊娠结局无关。结论 TNF-α基因238位点、IL-4基因-590位点基因多态性是导致梅毒妊娠孕妇阻断治疗失败的遗传学因素,但其与孕妇妊娠结局无关。OBJECTIVE To explore the association between the outcomes of blocking treatment and gene polymorphisms of immune-related factors in pregnant women with syphilis.METHODS The clinical data of 117 pregnant women with syphilis admitted to Wuhan Eighth Hospital from Jan.2017 to Oct.2020 were retrospectively analyzed,and all patients received blockade treatment.The polymorphisms of tumor necrosis factor α(TNF-α) gene at 238 locus and interleukin-4(IL-4) gene at-590 locus were detected by MGB TaqMan fluorescent probe method.The Hardy-Weinburg equilibrium goodness test was performed on each gene.According to clearance situations of syphilis after blockade treatment,they were divided into treatment success group(n=80) and treatment failure group(n=37).The genotypes and gene frequencies of TNF-α gene at 238 locus and IL-4 gene at-590 locus,and the incidence of adverse pregnancy outcomes were compared between the two groups.According to the pregnancy outcomes,they were divided into normal pregnancy group(n=103) and unfavorable pregnancy group(n=14).The genotypes and gene frequencies of TNF-α gene at 238 locus and IL-4 gene at-590 locus were compared between the two subgroups.The correlation between the gene polymorphisms of TNF-α gene at 238 locus and IL-4 gene at-590 locus and the outcome of syphilis blockade treatment outcomes and pregnancy outcomes was analyzed.RESULTS The distribution of G/A genotype in TNF-α gene at 238 locus and T/C genotype in IL-4 gene at-590 locus was accorded with Hardy-Weinberg equilibrium in both groups.The frequency of A in TNF-α gene at 238 locus and C in IL-4 gene at-590 locus in treatment success group were significantly higher than those in treatment failure group(P<0.05).The gene polymorphisms of TNF-α gene at 238 locus and IL-4 gene at-590 locus were related to the outcomes of blockade treatment in pregnant women with syphilis(P<0.05).The incidence of adverse pregnancy outcomes in treatment success group was significantly lower than that in treatment failure group(P<0.05).There we
