摘　　要：目的对医院临床科室线上初诊咨询系统的现状和随访结果进行描述和分析。方法采用描述性研究和单臂队列设计,以北京大学肿瘤医院乳腺癌预防治疗中心接受在线初诊咨询的连续病例为研究对象,收集初诊咨询系统处理类别和定期复查人群的随访资料,使用Mann-Whitney U检验比较组间差异。结果(1)线上初诊咨询系统有效处理比例为93.99%,直接约号类的中位年龄比定期复查类大10岁,差异有统计学意义(Z=-12.635,P﹤0.001);(2)通过初诊咨询系统为50.36%的患者进行约号处理,节约了中心8.97%的"门诊资源";(3)316人的定期复查固定队列中,36人(11.4%)完成了定期复查和线上二次咨询,其中8人(22.2%)需要门诊处理直接约号,完成和未完成定期复查人群的年龄特征差异无统计学意义(Z=-0.764,P=0.445)。结论医院临床科室线上初诊咨询系统具有一定优势和较好的推广应用价值。Objective To describe and analyze the circumstances and follow-up results of the online initial consultation system in clinical departments of hospitals.Method A descriptive study with one arm design was used to analyze the data of consecutive breast cancer patients who received online consultation in a breast center of Breast Cancer Prevention and Treatment Center,Beijing cancer hospital.Data of the treatment category and follow-up data of the Online Consultation System was collected.Mann Whitney U tests were used to compare the difference between groups.Result Firstly,the effective treatment rate of the online consultation system was 93.99%.The median age of the direct appointment group was 10 years older than that of regular review group,and the difference was statistically significant(Z=-12.635,P<0.001).Secondly,through the system,about 50.36%of the patients were processed,and 8.97%of the"outpatient resources"were saved.Thirdly,in the cohort of 316 people,36 people(11.4%)completed the regular review and online secondary consultation,of which 8 people(22.2%)needed outpatient treatment for direct appointment.The ages of the people who completed and did not complete the regular review were not statistically different(P=0.445).Conclusion It preliminarily confirmed the advantage of the online initial consultation system in the clinical departments of tumor hospital,and the system has good application value.
