通络开痹片联合玻璃酸钠治疗膝骨关节炎的临床研究

摘　　要：目的探讨通络开痹片联合玻璃酸钠注射液治疗膝骨关节炎的临床疗效以及安全性。方法选取134例膝骨关节炎患者,根据随机数表法将患者分为对照组和治疗组,每组67例。对照组给予玻璃酸钠注射液2 ml,每周1次。治疗组在对照组基础上口服通络开痹片,0.9 g/次,2次/天。两组均持续治疗6周。观察两组的临床治疗效果,比较两组治疗前后疼痛视觉模拟评分(VAS)、骨关节炎指数(WOMAC),炎性因子白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)及基质金属蛋白酶-3(MMP-3)水平的变化情况,观察记录治疗期间的不良反应。结果治疗后,治疗组的临床总有效率80.60%,显著高于对照组的62.69%(P<0.05);治疗后,两组患者VAS、WOMAC量表中关节疼痛、僵硬、日常生活功能评分及总评分较同组治疗前均显著降低(P<0.05),治疗组量表评分各项均显著低于对照组(P<0.05)。治疗后,IL-1β、TNF-α、IL-6及MMP-3水平均较治疗前显著下降(P<0.05),且治疗组均低于对照组(P<0.05)。结论通络开痹片联合玻璃酸钠注射液可改善膝骨关节炎患者关节功能,提高生活质量。Objective To investigate the clinical efficacy and safety evaluation of Tongluo Kaibi Tablets combined with sodium hyaluronate injection in the treatment of knee osteoarthritis.Methods 134 patients with knee osteoarthritis were selected and divided into control group and treatment group according to the random number table method,with 67 cases in each group.The control group was given 2 ml of sodium hyaluronate injection once a week.On the basis of the control group,the treatment group took orally Tongluo Kaibi Tablet,0.9 g/time,2 times/d.Both groups continued treatment for 6 weeks.Then the clinical treatment effects of the treatment group were compared with that of the control group,and the Changes of the pain visual analog scale(VAS),osteoarthritis index(WOMAC),inflammatory factors interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and matrix metalloproteinase-3(MMP-3)before and after treatment.And the adverse reactions during treatment were collected.Results After the treatment,the total clinical effective rate(80.60%)of the treatment group was significantly higher than that of the control group(62.69%,P<0.05);after treatment,the VAS and the scores of joint pain,stiffness and daily life function of WOMAC scales of the two groups were better(P<0.05),and the scale scores of the treatment group were significantly lower than those of the control group(P<0.05).After treatment,the levels of IL-1β,TNF-α,IL-6 and MMP-3 were significantly lower than those before treatment(P<0.05);and the levels of the treatment group were lower than those of the control group(P<0.05).Conclusion Tongluo Kaibi Tablet combined with sodium hyaluronate injection can improve joint function and quality of life in patients with knee osteoarthritis,and it is worthy of further clinical application.
