摘 要:目的探讨卡格列净对老年2型糖尿病(T2DM)合并心力衰竭患者的血糖及心功能的影响。方法在内分泌科及心内科2020年4月~2020年11月间收治的T2DM合并心力衰竭患者中抽取87例作为研究对象,按照随机数表法将87例入组患者分为对照组与研究组,对照组患者44例,接受常规降糖、抗心衰药物治疗,研究组43例,在与对照组相同治疗基础上联合应用卡格列净片,比较两组患者治疗前后炎症因子水平、血糖控制情况、心功能指标及不良预后。结果治疗后,研究组TNF-α、IL-6、hs-CRP、糖化血红蛋白、空腹血糖及餐后2 h血糖水平均低于对照组,BNP、LVEDP水平均低于对照组,LVEF、6MWT水平均高于对照组(P<0.05);不良预后总发生率(6.98%)低于对照组(34.09%),组间差异有统计学意义(χ^(2)=9.743,P=0.002)。结论卡格列净可抑制老年T2DM合并心力衰竭患者炎性反应,改善患者血糖水平、心功能与不良预后。Objective To analyze the effect of canagliflozin on blood glucose and cardiac function in elderly T2DM patients with heart failure.Methods From April 2020 to November 2020,87 case of T2DM patients with heart failure admitted to the Department of Endocrinology and Cardiology of our hospital were selected as the objects,and divided into control group and study group by random number table method,the control group patients received conventional hypoglycemic and anti heart failure medication,and the study group received canagliflozin tablets in combination with the same treatment as the control group,The levels of inflammatory factors,glycemic control,indexes of cardiac function and adverse outcomes were compared between the two groups before and after treatment.Results After the treatment,levels of TNF-α,IL-6,hs-CRP,glycated hemoglobin,fasting plasma glucose and 2h postprandial blood glucose were lower in the study group than those in the control group,levels of BNP and LVEDP in the study group were lower than those in the control group,levels of LVEF and 6MWT were higher than those in the control group(P<0.05),and the overall incidence of adverse outcomes(6.98%)was lower than that in the control group(34.09%),The difference between groups was statistically significant(χ^(2)=9.743,P=0.002).Conclusion In conclusion,canagliflozin can inhibit inflammatory responses and improve blood glucose levels,cardiac function and adverse outcomes in elderly patients with T2DM and heart failure.
