摘 要:目的探讨维生素D缺乏与绝经后女性盆底功能障碍的相关性。方法选择2018年3月~2020年12月就诊的绝经后女性178例作为对象,其中伴随有盆底功能障碍45例(障碍组),不伴随有盆底功能障碍133例(对照组),检测两组血清25-羟维生素D(25-OH-VitD)水平,判定维生素D缺乏状况并进行相关性分析。结果障碍组的血清25-OH-VitD含量低于对照组(P<0.05),维生素D正常率也低于对照组(P<0.05)。障碍组的尿失禁症状评分高于对照组(P<0.05),盆底肌力、盆底肌最大收缩力、盆底收缩持续时间少于对照组(P<0.05)。绝经后盆底功能障碍女性的尿失禁症状评分与25-OH-VitD、盆底肌力、盆底肌最大收缩力、盆底收缩持续时间均具有相关性(P<0.05);Logistic回归分析显示25-OH-VitD、盆底肌力、盆底肌最大收缩力、盆底收缩持续时间均为绝经后女性盆底功能障碍发生的重要影响因素(P<0.05)。结论绝经后盆底功能障碍女性多伴随有维生素D缺乏,尿失禁症状评分与血清25-OH-VitD水平存在相关性,血清25-OH-VitD水平也是导致绝经后女性发生盆底功能障碍的重要因素。Objective To explore and analyze the correlation between vitamin D deficiency and pelvic floor dysfunction in postmenopausal women.Methods From March 2018 to December 2020,178 cases of postmenopausal women treated in our hospital were selected as subjects that included 45 cases with pelvic floor dysfunction(disorder group)and 133 cases without pelvic floor dysfunction(control group).The serum 25-OH-VitD levels of the two groups were detected to analyze correlation of the vitamin D deficiency status and pelvic floor dysfunction.Results The serum 25-OH-VitD content of the disorder group were lower than that of the control group(P<0.05),and the normal rate of vitamin D of the disorder group were also lower than that of the control group(P<0.05).The score of urinary incontinence symptoms in the disorder group were higher than that of the control group(P<0.05),and the pelvic floor muscle strength,the maximum contractility of the pelvic floor muscles,the duration of pelvic floor contraction were shorter than those in the control group(P<0.05).The urinary incontinence symptom scores of postmenopausal women with pelvic floor dysfunction were correlated with 25-OH-VitD,pelvic floor muscle strength,pelvic floor muscle maximum contraction force and pelvic floor contraction duration(P<0.05);Logistic regression analysis showed 25-OH-VitD,pelvic floor muscle strength,maximum contractility of pelvic floor muscles and duration of pelvic floor contraction were all important factors influencing the occurrence of pelvic floor dysfunction in postmenopausal women(P<0.05).Conclusion Postmenopausal women with pelvic floor dysfunction are mostly accompanied by vitamin D deficiency.There are correlation between the urinary incontinence symptom score and the serum 25-OH-VitD level.The serum 25-OH-VitD level are also the key factor for causeing of pelvic floor dysfunction in postmenopausal women.
