中性粒细胞弹性蛋白酶遗传多态性与缺血性中风痰瘀证及临床风险指标的相关分析

摘　　要：目的探讨中性粒细胞弹性蛋白酶(ELANE)基因rs3761007及rs740021多态性与缺血性中风(IS)痰瘀证及其临床生物学指标的关联性。方法纳入IS痰瘀证组和对照组各550例,运用MassARRAY SNP基因分型技术进行基因分型,RT-PCR实验技术检测基因表达水平,PLINK软件完成遗传关联分析。结果 IS痰瘀证患者的ELANE基因mRNA表达水平与对照组之间的比较未见统计学意义(P>0.05),ELANE基因rs3761007、rs740021多态性与IS痰瘀证发生风险的关联均无统计学意义(均P>0.05);ELANE基因rs740021多态性与IS痰瘀证患者的APO-B(隐性模型:P_(adj)=0.007)、LDL(隐性模型:P_(adj)=0.004)、TC(隐性模型:P_(adj)=0.009)、APTT(隐性模型:P_(adj)=0.034)、FIB(显性模型:P_(adj)=0.007)、PT(隐性模型:P_(adj)=0.015)水平显著相关,而rs3761007多态性与IS痰瘀证患者的PLT水平显著关联(隐性模型:P_(adj)=0.033)。结论 ELANE基因rs3761007及rs740021多态性可能影响IS痰瘀证的血脂代谢及凝血功能,而ELANE基因多态性可能与IS痰瘀证的风险易感性无关。Objective To investigate the correlation between the polymorphisms of neutrophil elastase(ELANE) genes rs3761007 and rs740021 and the phlegm-stasis syndrome of ischemic stroke(IS) and its clinical biological indicators. Methods A total of 550 cases of IS with phlegm-stasis syndrome group and control group were included. MassARRAY SNP genotyping technique was used for genotyping, RT-PCR assay was used to detect gene expression level, and PLINK software was used for genetic association analysis. Results There was no statistical correlation between the mRNA expression level of ELANE gene in patients with IS with phlegm and blood stasis syndrome and the control group(P>0.05). There was no statistical correlation between rs3761007 and rs740021 polymorphism and the risk of IS with phlegm and blood stasis syndrome(P>0.05). ELANE gene rs740021 polymorphism was associated with APO-B level(recessive model: P_(adj)=0.007), LDL level(recessive model: P_(adj)=0.004), TC level(recessive model: P_(adj)=0.009), APTT level(recessive model: P_(adj)=0.034), FIB level(dominant model: P_(adj)=0.007) and PT level(recessive model: P_(adj)=0.015) in IS patients with phlegm and blood stasis syndrome was significantly correlated, while rs3761007 polymorphism was significantly correlated with PLT level(implicit model: P_(adj)=0.033). Conclusion The polymorphisms of ELANE genes rs3761007 and rs740021 may affect the lipid metabolism and coagulation function of IS patients with phlegm and blood stasis syndrome, while the polymorphism of ELANE gene may be independent of the risk susceptibility of IS with phlegm and blood stasis syndrome.
