加味交泰丸联合地衣芽孢杆菌活菌治疗腹泻型肠易激综合征临床疗效及对血清IL-6、IL-8、TNF-α水平影响

摘　　要：目的探讨加味交泰丸联合地衣芽孢杆菌活菌治疗腹泻型肠易激综合征肝郁脾虚证的疗效及对血清IL-6、IL-8、TNF-α水平影响。方法选取医院门诊收治的腹泻型肠易激综合征患者60例,随机分为两组各30例。对照组患者给予地衣芽孢杆菌活菌,实验组在对照组基础上给予加味交泰丸,两组患者均持续治疗28 d。对比两组患者的临床疗效、腹痛消失时间、大便性状、大便次数恢复正常时间及中医证候积分、IBS-SSS评分、IBS-QOL评分、血清IL-6、IL-8、TNF-α水平及不良反应情况。结果治疗后,实验组总有效率较高(P<0.05);实验组腹痛消失时间、大便性状、大便次数恢复正常时间明显低于对照组(P<0.05);两组患者治疗后腹痛、大便性状、大便次数等证候评分显著降低(P<0.05);实验组降低较明显(P<0.05);两组治疗前血清IL-6、IL-8、TNF-α水平差异无统计学意义(P>0.05);两组治疗后血清IL-6、IL-8、TNF-α水平显著降低(P<0.05);实验组降低较明显(P<0.05);两组治疗前IBS-SSS评分差异无统计学意义(P>0.05);两组治疗后IBS-SSS评分显著降低(P<0.05);并且实验组降低较明显(P<0.05);两组患者治疗前IBS-QOL评分无差异(P>0.05);两组患者治疗后IBS-QOL量表各条目评分显著降低(P<0.05);并且实验组降低较多(P<0.05)。结论采用加味交泰丸联合地衣芽孢杆菌活菌治疗腹泻型肠易激综合征肝郁脾虚证具有较好的治疗效果,能够降低血清IL-6、IL-8、TNF-α水平,安全性较高,值得在临床上推广应用。Objective To investigate the therapeutic effect of Jiawei Jiaotai Pill(加味交泰丸)combined with live Bacillus li-cheniformis in the treatment of liver depression and spleen deficiency syndrome of diarrhea-type irritable bowel syndrome(IBS)and its effect on serum levels of IL-6,IL-8 and TNF-α.s Methods Sixty cases of diarrhea-type IBS admitted to our hospital outpatient were randomly divided into two groups with 30 cases in each.Patients in the control group were given live Bacillus li-cheniformis,and those in the experimental group were given Jiawei Jiaotai Pill on the basis of the control group.Both groups were treated continuously for 28 d.The clinical efficacy,time for abdominal pain to disappear,stool characters,time for stool fre-quency to return to be normal,TCM syndrome score,IBS-SSS score,IBS-QOL score,serum levels of IL-6,IL-8 and TNF-αand adverse reactions were compared between the two groups.s Results After treatment,the total effective rate of the ex-perimental group was higher(P<0.05).The time for abdominal pain to disappear,stool character and stool frequency to return to be normal in the experimental group were significantly lower than those in the control group(P<0.05).After treatment,symptoms such as abdominal pain,stool character and stool frequency were significantly reduced in both groups(P<0.05).In the experimental group,the decrease was more significant(P<0.05).There was no difference in serum levels of IL-6,IL-8 and TNF-αbetween the two groups before treatment(P>0.05).After treatment,serum levels of IL-6,IL-8 and TNF-αwere significantly decreased in both groups(P<0.05).In the experimental group,the decrease was more significant(P<0.05).There was no difference in IBS-SSS scores between the two groups before treatment(P>0.05).After treatment,the degree of abdominal pain,frequency of abdominal pain,degree of abdominal distension,satisfaction with defecation and the in-fluence score on life in both groups were significantly reduced(P<0.05).In the experimental group,the decrease was mor
