加味清胃汤联合盐酸米诺环素治疗慢性牙周炎的疗效及对龈沟液IL-1β、IL-6和PGE_(2)水平影响

摘　　要：目的探讨加味清胃汤联合盐酸米诺环素治疗慢性牙周炎的疗效及对龈沟液IL-1β、IL-6和PGE_(2)水平影响。方法选取医院收治的慢性牙周炎患者214例,随机分为两组各107例。对照组患者给予盐酸米诺环素软膏,实验组在对照组基础上给予加味清胃汤,两组患者均持续治疗28 d。对比两组患者的临床疗效、牙周指标水平、中医证候评分、龈沟液IL-1β、IL-6和PGE_(2)水平、SF-36量表评分及不良反应情况。结果治疗后,实验组总有效率较高(P<0.05);两组治疗前组牙周指标水平分差异无统计学意义(P>0.05);两组治疗后PLI、GI、PD、SBI等牙周指标水平显著降低(P<0.05);并且实验组降低较多(P<0.05);两组治疗前中医证候评分差异无统计学意义(P>0.05);两组治疗后牙龈肿痛、口臭、口干舌燥、齿衄、便秘评分显著降低(P<0.05);并且实验组牙龈肿痛、口臭、口干舌燥、齿衄、便秘评分明显低于对照组(P<0.05);两组治疗前龈沟液IL-1β、IL-6和PGE_(2)水平差异无统计学意义(P>0.05);两组治疗后龈沟液IL-1β、IL-6和PGE_(2)水平显著降低(P<0.05);并且实验组降低较明显(P<0.05);两组治疗前SF-36量表评分差异无统计学意义(P>0.05);两组患者治疗后SF-36量表评分明显升高(P<0.05);并且实验组升高较明显(P<0.05)。结论采用加味清胃汤联合盐酸米诺环素治疗慢性牙周炎具有较好的治疗效果,能够降低龈沟液IL-1β、IL-6和PGE_(2)水平,安全性较高,值得在临床上推广应用。e Objective To investigate the efficacy of Jiawei Qingwei Decoction(加味清胃汤)combined with minocycline hydrochloride in the treatment of chronic periodontitis and its effect on the levels of IL-1β,IL-6 and PGE 2 in gingival crevicu-lar fluid.s Methods A total of 214 patients with chronic periodontitis admitted to our hospital were randomly divided into two groups:107 patients in each group.Patients in the control group were given minocycline hydrochloride ointment,while those in the experimental group were given Jiawei Qingwei Decoction on the basis of the control group.Patients in both groups were trea-ted continuously for 28 days.Clinical efficacy,periodontal index level,TCM syndrome score,levels of IL-1β,IL-6 and PGE 2 in g ingival crevicular fluid,SF-36 scale score and adverse reactions were compared between the two groups.s Results After treatment,the total effective rate of the experimental group was higher(P<0.05).There was no difference in the level of perio-dontal index between the two groups before treatment(P>0.05).After treatment,the levels of periodontal indexes such as PLI,GI,PD and SBI in the two groups were significantly decreased(P<0.05).And the reduction in the experimental group was sig-nificant(P<0.05).There was no difference in scores of TCM syndromes before treatment between the two groups(P>0.05).After treatment,the scores of gingival swelling and pain,halitosis,epistaxis and constipation were significantly reduced in both groups(P<0.05).In addition,the scores of gingival swelling and pain,halitosis,dry mouth,epistaxis and constipation in the experimental group were significantly lower than those in the control group(P<0.05).The levels of IL-1β,IL-6 and PGE_(2) in gingival crevicular fluid were not different between the two groups(P>0.05).The levels of IL-1β,IL-6 and PGE_(2) in gin-gival crevicular fluid were significantly decreased after treatment in both groups(P<0.05).In the experimental group,the de-crease was more significant(P<0.05).There was no difference in SF-36 scale sc
