清肺平喘汤联合沙美特罗替卡松治疗小儿支气管哮喘的疗效及对免疫功能影响

摘　　要：目的探讨清肺平喘汤联合沙美特罗替卡松治疗小儿支气管哮喘的疗效及对免疫功能影响。方法选取医院收治的支气管哮喘患儿64例,随机分为两组各32例。对照组患儿雾化吸入沙美特罗替卡松,实验组在对照组基础上给予清肺平喘汤,两组患儿均持续治疗14 d。对比两组患儿的临床疗效、临床症状消失时间、中医证候评分、FEV_(1)、FEV_(1)/FVC、PEF值、血清IL-6、TNF-α和CRP水平、免疫功能指标及不良反应情况。结果治疗后,实验组总有效率较高(P<0.05);实验组患者胸闷、咳嗽、喘息、哮鸣音等症状消失时间明显低于对照组(P<0.05);两组治疗前FEV 1、FEV 1/FVC、PEF值差异无统计学意义(P>0.05);两组治疗后FEV 1、FEV 1/FVC、PEF值显著升高(P<0.05);并且实验组升高较明显(P<0.05);两组治疗前血清IL-6、TNF-α和CRP水平差异无统计学意义(P>0.05);两组治疗后血清IL-6、TNF-α和CRP水平显著降低(P<0.05);并且实验组降低较明显(P<0.05);两组治疗前中医证候评分差异无统计学意义(P>0.05);两组治疗后咳嗽、喘息、胸闷、咳痰等证候评分显著降低(P<0.05);并且实验组降低较多(P<0.05);两组治疗前血清CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平差异无统计学意义(P>0.05);两组患儿治疗后血清CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平明显升高(P<0.05);并且实验组升高较明显(P<0.05)。结论采用清肺平喘汤联合沙美特罗替卡松治疗小儿支气管哮喘具有较好的治疗效果,能够提高患儿免疫功能,安全性较高,值得在临床上推广应用。e Objective To investigate the therapeutic effect of Qingfei Pingquan Decoction(清肺平喘汤)combined with salme-teroticasone on bronchial asthma in children and its influence on immune function.s Methods Sixty-four cases of children with bronchial asthma admitted to our hospital were randomly divided into two groups with 32 cases in each.Children in the control group received atomized inhalation of salmeteroticasone,and those in the experimental group were given Qingfei Pingquan Decoc-tion on the basis of the control group.Children in both groups were treated continuously for 14 days.The clinical efficacy,disap-pearance time of clinical symptoms,TCM syndrome score,FEV 1,FEV 1/FVC,PEF,serum levels of IL-6,TNF-αand CRP,immune function indicators and adverse reactions were compared between the two groups.s Results After treatment,the total effec-tive rate of the experimental group was higher(P<0.05).The disappearance time of symptoms such as chest tightness,cough and wheezing in the experimental group was significantly lower than that in the control group(P<0.05).There was no difference in FEV 1,FEV 1/FVC or PEF between the two groups before treatment(P>0.05).The values of FEV 1,FEV 1/FVC and PEF in the two groups were significantly increased after treatment(P<0.05).The increase in the experimental group was more signifi-cant(P<0.05).There was no difference in serum levels of IL-6,TNF-αor CRP between the two groups before treatment(P>0.05).After treatment,serum levels of IL-6,TNF-αand CRP in the two groups were significantly decreased(P<0.05).In the experimental group,the decrease was more significant(P<0.05).There was no difference in scores of TCM syndromes before treatment between the two groups(P>0.05).After treatment,the scores of cough,wheezing,chest tightness,ex-pectoration and other symptoms in the two groups were significantly reduced(P<0.05).In addition,there was a significant de-crease in the experimental group(P<0.05).There was no difference in serum levels of CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(
