摘　　要：目的基于真实世界研究探析纯中药治疗2型糖尿病(T2DM)的临床疗效及对胰岛功能的影响。方法收集自2018年8月—2020年8月间来自5家医疗机构的T2DM患者252例,采用纯中药"序贯三法"治疗,12周为1疗程,观察治疗前后空腹血糖(FBG)、餐后2 h血糖(2hPG)、空腹及餐后1 h、2 h、3 h胰岛素水平,空腹及餐后1 h、2 h、3 h胰高血糖素水平,胰岛β细胞功能(HOMA-βF)水平改善情况。结果本研究受试者252例,治疗后空腹血糖(FBG)、餐后2 h血糖(2hPG)均显著下降,差异具有统计学意义(P<0.01);空腹及餐后1 h、2 h胰岛素水平较治疗前明显升高,差异具有统计学意义(P<0.01)。餐后3 h胰岛素水平无明显变化,差异无统计学意义(P>0.05);空腹及餐后1 h、2 h、3 h胰高血糖素水平较治疗前明显下降,差异具有统计学意义(P<0.01);胰岛β细胞功能(HOMA-βF)较治疗前明显上升,差异具有统计学意义(P<0.01)。结论纯中药"序贯三法"治疗T2DM临床疗效确切,能有效降低血糖、改善胰岛素分泌,降低胰岛素抵抗,降低胰高血糖素,改善患者胰岛功能,疗效确切,值得临床推广。Objective To observe the clinical efficacy of pure Chinese medicine in the treatment of type 2 diabetes mellitus and its effect on islet function based on the real world research.Methods A total of 252 patients with type 2 diabetes who met the diagnostic criteria were collected from 5 Chinese hospitals from August 2018 to August 2020. They were treated with pure Chinese medicine "sequential three methods" for a course of 12 weeks.Fasting blood glucose(FBG), 2 h postprandial blood glucose(2 hPG), fasting and 1 h, 2 h, 3 h postprandial insulin levels, fasting and 1 h, 2 h, 3 h postprandial glucagon levels, and islet β-cell function(HOMA-βF) levels were observed before and after treatment.Results A total of 252 subjects were included in this study. After treatment, both FBG and 2 hPG were decreased significantly(P<0.01).The fasting and 1 h and 2 h insulin levels were significantly higher than those before treatment(P<0.01).There was no significant difference in insulin level at 3 h after meals(P>0.05).The levels of glucagon in fasting and 1 h, 2 h and 3 h after meals were significantly lower than those before treatment(P<0.01).The function of HOMA-βF was significantly increased compared with that before treatment(P<0.01).Conclusion The pure traditional Chinese medicine "sequential three methods" has a definite clinical effect on the treatment of type 2 diabetes mellitus, which can effectively reduce blood glucose, improve insulin secretion, reduce insulin resistance, and improve the function of islet β-cell in patients. The curative effect is definite and it is worthy of further clinical promotion.
