超声引导下神经根阻滞配合龙氏正骨推拿手法治疗神经根型颈椎病的临床研究

摘　　要：目的:观察超声引导下神经根阻滞配合龙氏正骨推拿手法治疗神经根型颈椎病的临床疗效。方法:纳入2019年1~2019年12月广东省中西医结合医院康复科收住院的神经根型颈椎病患者60例,利用随机数字表法分为治疗组和对照组各30例,在常规健康宣教及颈部功能锻炼指导的基础上,对照组采用超声引导下神经根阻滞治疗,治疗组则在此基础上加用龙氏正骨推拿手法治疗,采用视觉模拟评分法(VAS)和颈椎功能障碍指数(NDI)分别对两组患者治疗前、治疗后第1天、治疗后第7天和疗程结束后3个月4个时点进行疗效评估。结果:治疗组VAS评分、NDI评分改善程度均优于对照组(P<0.01);治疗组治疗后第7天、3个月优良率均高于对照组(P<0.05)。结论:超声引导下神经根阻滞配合龙氏正骨推拿手法治疗神经根型颈椎病疗效更显著,可安全且持久地减轻患者痛苦,恢复患者得颈椎功能。Objective:To observe the clinical effect of ultrasound-guided nerve root block combined with Long’s bone setting massage in the treatment of cervical spondylotic radiculopathy.Methods:A total of 60 patients with cervical spondylotic radiculopathy who were admitted to the rehabilitation department of Guangdong Provincial Hospital of Integrated Traditional Chinese and Western Medicine from January 2019 to December 2019 were included,and randomly divided into treatment group(n=30) and control group(n=3 0) by random number table method.On the basis of routine health education and neck function exercise guidance,the control group was treated with ultrasound-guided nerve root block treatment,while treatment group combined with Long’s bone setting massage on the basis of treatment in control group,Visual Analogue Scale(VAS) and Neck Disability Index(NDI) were used to evaluate the efficacy of the two groups at the times of before treatment and 1 st day,7 th day,3 months after treatment.Results:The improvement of VAS and NDI scores in treatment groups were better than those in control group(P<0.01);the 7 th day and 3 months after treatment,the excellent and good rate of treatment group were higher than those of control group(P<0.05).Conclusion;Ultrasound-guided nerve root block combined with Long’s bone setting massage is more effective in the treatment of cervical spondylotic radiculopathy,which can safely and permanently reduce the pain and restore the cervical function of patients.
